Case No.s; GE02015-00034, MEP2015-00033, SEP2015-00059, SHX2015-00109, BLD2015-00673 I was contacted by Mr. Rich Kennedy of"Integrated NW Construction" (253-888-5314)earlier this month to inform me that he would be setting up his pile driving equipment the following week and that he planned to start driving the piles for the Martineau bulkhead on week of November 13 thru the 17`h. Equipment breakdowns delayed the project for most of the week, and the bulkhead and pier positioning pile were driven the week of November 20h through the 24`h. I visited the site Tuesday morning and witnessed the installation of a majority of the piles called for in the approved design. The approved building permit included the following requirement; Piles/Pile Driving Owner shall provide pile drivers report on all piles.Any pile supporting structure and over 10'in height from floor of body of water shah require engineered design and special inspection. Engineer of record(or certified special:inspector)shall provide verification at time of inspection that piles are driven to point of refusal and/or installed per design.X The pile driver was a hydraulic vibratory attachment mounted to a Kubota excavator pictured on the next page. The average pile driving depth to refusal (Less than 1"advancement in 1 minute)ranged between 5 to 7 feet. The piles were driven to refusal and cut off at their design elevation. Two pile diameters were installed. Site Development& Landowner Concerns: The Martineaus retained Integrated NW Construction to replace a portion of an existing bulkhead and other features as identified in Integrated NW Construction's Scope of Work(attached)as well as the project drawings produced by myself dated 2/3/2023 (attached). The work complies with MCC 17.50.340 B.2 a.i,and a.iii in that the bulkhead provides protection to the residence on the property and the septic system components,and that it provides the only access associated with the principal use of the property that cannot be feasibly relocated. Due to the deterioration of the wood piles and pier/deck components,the Martineaus are concerned about safety and the use of the shoreline features to access lake Cushman. PO Box 235,I Ioodsport, WA 98548 (360)877-9512 The attached plans provide the locations of the existing features as well as the completed repairs and replacement items.The existing residence(Built 1983)is roughly 70' horizontally upland from the existing bulkhead with an average downslope of 65°.No wetlands or surface water drainage system adverse issues were identified. Site Vegetation: The site vegetation has been planted and maintained as landscaping with naturally occurring vegetation surrounding the residence. Discussion of Geologic Conditions: The site is located on the easterly shore of Lake Cushman Reservoir approximately 5 miles west of Hoodsport,Washington on the Olympic Mountain foothills. The general slope of the property from Marine View Loop to the top of the bulkhead is approximately 65°. Probing with a 4' rod found a moderately dense loamy sand(weathered till) covering most of the site to a depth between 3 &4' deep. Driven pipe-piles were typically driven to refusal at 14'.Probe locations and depths to the underlying firm hard soil layer are shown on plan sheets 3&5. The Hoodsport soils are not listed under Mason County's Resource Ordinance as an erodible soil. Inspection of the Washington Coastal Atlas, landslide hazard section, found the site located in an area identified as"stable". The project area lies at the base of the Olympic Mountain Foothills which can be characterized as a mix of ravines,valleys,and a gentle sloping shelf which connects the foothill plateau with the steep Olympic Mountains to the west. The immediate upslope geomorphology of the Martineau property may be described as"hilly". Assessment of Regional/Site Geology including the influence of Wave or Tidal Processes: The Martineau property and residential properties surrounding the site display no evidence of recent slope instability or earth movement.The Martineau and surrounding properties are constantly subject to erosion from wave and"tidal action"—annual raising and lowering of the lake's surface water elevation between—710 feet sea level and 738 sea level". With the shoreline being annually submerged and then exposed,the freeze thaw environment erodes the entire shoreline and roughly 1 to 2"of shoreline elevation is eroded away each year. Evaluation of Erosion or Flooding Issues: The residence was built in 1983 and has not been subject to any erosion or flooding issues other than the shoreline and bulkhead deterioration. Evaluation of the Amount of Time until Primary Structures or Major Utilities will be damaged by Wave or Tidal Action: If the bulkhead and slope behind the bulkhead are not maintained,the bulkhead could fail within a couple of years. The existing septic system was installed 100 feet landward of the Lake Cushman shoreline,and any loss of ground between the existing bulkhead and septic drain field would create a potential public health hazard. Discussion of Alternative Approaches to reduce the risk from Erosion on the site: The bulkhead could benefit from having logs anchored along the toe of the existing bulkhead,no"soir' annoring erosion control features have been found to be successful. Discussion Describing the proposed shoreline stabilization is the minimum Necessary: The shoreline armoring being repaired has lasted several decades,and unless it is allowed to remain with routine maintenance,the existing residence could fail as well as the existing septic system. I should have circled back with you after I talked to Tom. I told Tom that the most important thing for TPU in the dock plans that were approved was the reduction of overwater coverage to 340 sf(See attached app review). This allows for flexibility to deal with the plausible issues with the overlapping dock and float. So at the end of the day, if the total overwater coverage is 340 sf or less (this includes the float), then it would be ok with Tacoma. I told him the easiest thing would be to follow the approved plans because it is already permitted. I told him if he wants to change the plans the get down to 340 another way, then it would require going back to the regulatory agencies for changes to the permits and Tacoma for an amendment to the Construction License. I also let him know that the construction window generally closes in early April as so he doesn't have a lot of time. We talked about the fixed section overlapping the float and the concerns with the new flotation potentially causing damage/being damaged during full pool. He mentioned wanting to wait to see how the float reacts to full pool, but I reminded him that his permits did not extend that long and that in-water work is (generally) not allowed. This is especially a problem for him as his float cannot be removed. Let me know what he decides to do. He is under the impression that if he removes the I-shaped portion of the fixed pier where it overhangs the float that you'll approve less reduction in the total length. I told him that I did not i interpret your letter that way, nor did I get that impression from speaking with you on the phone about this project. I've updated Rich about this. He has been sick for the last week or so. He said he'll call Tom tomorrow to go over this with him. Rich is going to tell him that he is not correct-we need to reduce the fixed pier to the total length that was approved in the drawings,which is 9.7 feet. He'll let Tom know to also reduce the I-shaped section where it overhangs the float will require a revision with Mason County and WDFW. Did I get that correct? 3. Proper disposal of construction debris must be on land in such a manner that debris cannot enter or cause water quality degradation of State waters. Proper erosion and sediment control practices must be used on the construction site and adjacent areas to prevent upland sediments from entering shoreline waters. Erosion control measures must be in place prior to any clearing, grading, or construction. Proper disposal of construction debris must be on land in such a manner that debris cannot enter or cause water quality degradation of State waters. Proper erosion and sediment control practices must be used on the construction site and adjacent areas to prevent upland sediments from entering shoreline waters. Erosion control measures must be in place prior to any clearing, grading, or construction. These control measures must be effective to prevent soil from being carried into surface water by stormwater runoff. Sand, silt, and soil will damage aquatic habitat and are considered pollutants. Any discharge of sediment-laden runoff or other pollutapollutants to waters of the state is in violation of Chapter 90.48 RCW, Water Pollution Control, and WAC 173-201A, Water Quality Standards for Surface Waters of the State of Washington, and is subject to enforcement action. Any work in or adjacent to waterways that will adversely affect water quality must receive specific prior authorization from the Department of Ecology pursuant to WAC 173-201A-110. A short-term water quality standards modification may be issued if the proponent agrees to a number of specific construction practices and techniques designed to minimize water quality impacts. All areas disturbed or newly created by construction activities must be revegetated using bioengineering techniques, clean durable riprap, or some other equivalent type of protection against erosion when other measures are not practical. * A Hydraulic Project Approval from the Washington Department of Fish and Wildlife shall be obtained prior to work on the banks of or within the waters of the State. " All required State and Federal permitting must be obtained prior to commencing work on site. • Per the Biological Evaluation prepared by Kim Schaumburg of BioResources, LLC and dated July 31, 2019, six native trees or large shrubs will be planted along the shoreline where it is lacking. The trees or shrubs should be acquired using the recommendations set forth in the Biological Evaluation. Planting, monitoring, and mitigation should also follow the outline of the before mentioned report. It is the property owner's responsibility to submit to the County a monitoring report containing pictures of the required plants and their location in the shoreline buffer(site plan). Monitoring period is for three years from the date of planting. The first report with pictures should be submitted to the County right after planting. Property owner shall provide pictures that are taken against a measuring device or against an object that can be used to reference scale as the monitoring process progresses. OWNER/BUILDER acknowledges submission of inaccurate information may result in a stop work order or permit revocation. Acknowledgement of such is by signature below. All construction must be in conformance with the international codes as amended and adopted by Mason County.
