ENGINEER SITE VISIT/PRE-POUR REPORT
Project Name: Mason County Waste Export
Transfer Station
PMX #21-1682-16
Report Prepared By: Kevin J. Dour, P. E.
Project Engineer
Contractor: Pease Construction
Date: July 27 , 1993
Contractor Personnel On-Site: Dan Bloczynski - Superintendent
Ken Bapp - Carpenter
George Nelson - Carpenter
James Longshore - Carpenter
Elvin Walker - Carpenter
Jeff Showse - Carpenter Appr.
Rich Webster - Laborer
Carbek Steel - 3 Ironworkers
Other Personnel Present: None
Equipment Observed On-Site: 6000# Rough Terrain Fork Lift
Gas Generator
Miscellaneous Hand Tools
Materials observed On-Site: Concrete Forms
Rebar for Walls including
column ties and wall dowels,
and crane plinth
Misc. framing lumber, Trench
drains, wall forming material.
Inspection:
During the visit the following items were checked or observed:
1. The as-builts for the project were inspected. They appeared
to be legible and complete. The changes for Change Order No.
1 were shown along with all other minor changes in the project
to date.
2. The walls along grid 1 and 3 were inspected. The bars were
measured for distance on-center, the depth of concrete from
face of wall to edge of bar, and the bar type. These check
with what is required according to the plans and
specifications. They were in the process of installing one
expansion joint along this grid. They were cutting every
other horizontal bar according to what is required. The forms
for these wall were still being installed at the time of the
inspection.
3 . The wall along grid B was inspected. Most of the bars and
forms were in place. The bars measured the correct distance,
12 inches on-center and were the correct type. There are five
expansion joints required along this wall. They were
installed and the horizontal bars wore cut, every other one,
according to what is required. The depth of concrete from the
face of wall to the edge of bar measured to what is required
in the plans.
4 . The distance from the face of the crane plinth to the face of
the wall along grid B was measured. This measured at 15 feet
9 inches. This is adequate since the required distance from
the plans is 15 feet.
5. The concrete pour for the footing for the wall along grid A
was inspected. The additional 18 inches for the two column
footings for the building were formed according to what was
requested in the last inspection. These footings were located
between grids 1 and 2 and grids 2 and 3 .
6. The footings between grids B and D along grids 1 and 3 were in
progress. The steel for the footings was not placed yet.
This will be included in the pour scheduled this week. I
asked Dan if he could have Tom or Paul look at the steel
before the pour to verify if it was placed according to what
is required in the plans. I also asked if he could have Tom
or Paul photograph the steel for record.
Some additional questions Dan had during the site visit:
1. Is any grout required underneath the plates for the crane
plinth or underneath the plates for the building column
footings? He did not notice it on any of the details.
[GROUTING IS REQUIRED]
2 . Dan had a question on the detail on the Anchor Bolt Setting
Plan. This is a drawing we had not yet received as a
submittal. The question was on a callout for "shear angle as
required" . This shear angle is used to stabilize the anchor
bolts and is embedded in the concrete. Dan was wondering what
size is required so he can order the angle iron. He gave me
a copy of a facsimile he received of the drawing.
[PARAMETRIX IS WORKING ON THIS QUESTION]
3 . The specifications require that anchor bolts and embedded
products need to be made out of a material that is "compatible
with an embedded concrete environment. " Normally what Dan
uses is a steel bolt which is what he ordered. He was
wondering if the specs meant that a galvanized bolt is
required instead.
[GALVANIZED STEEL BOLTS PER BUILDING MANUFACTURER' S SIZE
REQUIREMENTS ARE REQUIRED. THE BOLTS NEED TO BE GALVANIZED
DUE THE FACT THEY ARE PARTIALLY EXPOSED]
4 . Dan brought up a concern regarding inspections from the Mason
County Building Department. He has called for an inspection
more than once and they have not come out. He is aware of the
conversation between Randy and the County Building Inspector
which indicated that Randy' s inspection would be fine for the
inspection requirement. He just wanted to let me know that he
is going through the required channels by asking for an
inspection. He wanted to be sure there were not going to be
any surprises down the road.
Additional Notes:
1. This inspection was for the pour which will occur on Thursday,
July 29th or Friday, July 30th. This will include the wall
along grid B, the walls along grid 1 and 3 about 13 feet back
from grid B, and the footings between grids C and D along
grids 1 and 3 .
2 . According to Dan, the next pour should include the wall along
grid A, the Crane Plinth, and the walls remaining along grids
1 and 3 . This pour should occur on the following Thursday,
August 5th.
3 . Pay Estimate for the concrete retaining walls claiming 55%
percent of the work being complete seems to be accurate based
upon the work observed at the job site. The overall total of
24% of contract amount seems reasonable. We recommend
acceptance of the pay estimate.
