1) Owner/Agent is responsible to post the assigned address and/or purchase and post private road signs in accordance with Mason County Title
14.28.

2) ALL CONSTRUCTION MUST MEET OR EXCEED ALL LOCAL CODES AND THE INTERNATIONAL CODE REQUIREMENTS AND
OCCUPANCY IS LIMITED TO THE PERMITTED AND APPROVED CLASSIFICATION. ANY CHANGE OF USE OR OCCUPANCY WOULD
RESULT IN PERMIT REVOCATION. CHANGE OF USE MUST BE APPROVED PRIOR TO CHANGE. x //c>
3) Changes to approved building plans that affect compliance to the current Washington State Energy Code (WSEC), ventilation requ ire ments),
Building/Plumbing/Mechanical Codes and/or Mason County Regulations shall be approved prior to construction.

4) Any changes in construction shall be reviewed by engineer of record and submitted in writing to the Mason County Building Department prior to
construction. All engineering documents are a part of the approved set of plans and must remain attached thereto. If engineering documents are
removed, approval will not be granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour will be charged and
collected by the Mason County Building Department prior to any further inspections being performed or approvals granted.

5) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE
ADOPTED BUILDING CODE.
The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in
conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a
Mason County Building Inspector shall be made prior to requesting additional inspections.

6) • All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The
failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being
non-compliant with Mason County ordinances and building regulations.

7) Install 3A20BC fire exinguishers throughout the building with a maximum travel distance of 75 feet in any direction and monuted no more than 60
inches above the floor to the top of the unit.

Install a knox box on all buildings and gates per section 506 of the 2009 International Fire code. Please contact the local fire district for more
information and inspections.

An NFPA 72 compliant fire alarm system is required to be installed. The system is required to be fully monitored by a UL certified monitoring
company. A separate pet application is required to be submitted and approved prior to the installation of the system.

8) PER TITLE 14 MASON COUNTY BUILDING CODE-CHAPTER 14.17, STANDARDS FOR FIRE APPARATUS ACCESS ROADS - 14.17.110:
A fire apparatus access road in excess of 14% grade and more than 150'to new residential or commercial structures will require an automatic fire
sprinkler system installed. Contact the Mason County Fire Marshal at(360)427-9670, extension 352, for further information.

9) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the
time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control
of the permit holdeE have prevented action from being taken. No more than one extension may be granted.

10) All approved plans are required to be on-site for inspection purposes. If inspection is called for and plans are not on site, Approval WILL NOT be
granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour will be charged and collected by the Mason County
Building Department prior to any further inspections being performed or approvals granted. X
11) All property lines shall be clearly identified at the time of foundation inspection. X
12) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance
Division. There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be
obtained at 1-800-647-0982. The person signing this condition is either the homeowner, agent for the owner or a registered contractor according to

13) The approved site plan is required to be on-site for inspection purposes. If inspection is called for and the site plan is not on site, Approval WILL

14) . Per section 6.0 "Closure" of the Soils report dated March 22, 2010 prepared by Michael Staten P.E.
Due to the inherent natural variations of the soils stratification and the nature of the geotechnical subsurface explorations, , there is aways a
possiblity that soil conditions encountered during construction are different than those described in this report. Therefore, it is recommended that a
qualified engineer observes and documents the construction, or envirotech is promptly notified if project and subsurface condition found on site are
not as presented in this report so that we can re-evaluate our recommendations.
Mason County is requiring to be done, as a condition of the permits issuance.

15) An inspection performed by a representative of the geologist, their authorized representative, or engineer of record, shall be completed on-site prior
to the footing pour to verify that earthwork and foundation installation activities comply with the Geotechnical report or assessment
recommendations.
In addition a final inspection report, prepared by a representative of the geologist, their authorized representative, or engineer of record shall be
required to verify that erosion control, drainage and final work is completed in accordance to the Geotechnical report or assessment
recommendations.
Reports shall be submitted to the Mason County Building Dept., PO Box 186, Shelton, WA 98584, prior to the footing and final inspections and
available for inspection.

16) Compliance with an approved Storm Water Plan shall be subject to an on site inspection, or inspections, by the engineer of record or an
authorized representative. Said inspection(s) shall be performed in accordance with the approved plan, and prior to any modification that would
make a determination of compliance impossible.
Inspection reports shall be submitted to verify all engineering requirements have been completed in accordance with the approved storm water
plan. Reports shall be submitted to the Mason County Department of Community Development prior to each corresponding inspection and final
permit approval. Copies of all special inspection reports shall be made available at time of inspection.

17) All clearing, grading, vegetation removal or development shall be greater than 150 feet from the ordinary high water mark of the stream. A Mason
Environmental Permit and Habitat Management Plan (mitigation) is required in order to remove vegetation or otherwise disturb the buffer.

18) Water quality is not to be degraded to the detriment of the aquatic environment as a result of this project.

19) Approved per dimensions and setbacks on submitted site plan. Setbacks are measured from the furthest projection of the structure.

20) The approval of this project is subject to the recommendations and specifications outlined in the attached geotechnical report prepared by Michael
Staten of Envirotech Engineering, dated February 28, 2011. Structures and/or land modifications (grading, cuts, fills, etc.) required in the
geotechnicaleport, may require a seperate permit. The geotechincal report shall remain attached to the approved building plans.
This permit becomes null and void if work orconstruction authorized is not commenced within 180 days,or if construction or work is suspended for a period of 180 days at any time after work is
commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied. Proof of continuation of
work is by means of a progress inspection.The owner or the agent on the owners behalf, represents that the information provided is accurate and grants employees of Mason County access to
