An environmental impact statement (EIS) must be prepared for all proposals with probable significant adverse impacts on the quality of the environment. The purpose of this checklist is to provide information to help you and the agency identity impacts from your proposal (and to reduce or avoid impacts from the proposal, if it can be done) and to help the agency decide whether an EIS is required. Instructions for Applicants: This environmental checklist asks you to describe some basic information about your proposal. Governmental agencies use this checklist to determine whether the environmental impacts of your proposal are significant, requiring preparation of an EIS. Answer the questions briefly, with the most precise information known, or give the best description you can. You must answer each question accurately and carefully, to the best of your knowledge. In most cases, you should be able to answer the questions from your own observations or project plans without the need to hire experts. If you really do not know the answer, or if a question does not apply to your proposal, write "do not know" or "does not apply". Conditions of this Case are listed below: Case No.:SEP99-0015 1) Water quality is not being degraded to the detriment of the aquatic environment as a result of this project. 2) A Hydraulic Project Approval from the Washington State Department of Fish & Wildlife must be granted prior to construction. X 3) Proper erosion and sediment control practices must be used on the construction site and adjacent areas to prevent upland sediments from entering shoreline waters. Erosion control measures must be in place prior to any clearing, grading, or construction. These control measures must be effective to prevent soil from being carried into surface water by stormwater runoff. Sand, silt, and soil will damage aquatic habitat and are considered pollutants. 4) All upland areas disturbed or newly created by construction activities shall be seeded, vegetated or given some other equivalent type of protection against erosion. 5) The proposed project must be consistent with all applicable policies and other provisions of the Shoreline Management Act, its rules, and the Mason County Shoreline Master Program. X 6) Proper disposal of construction debris must be on uplands, in such a manner that debris cannot enter wetlands or cause water quality degradation of adjacent waters. 7) All construction and demolition debris must be removed from the beach after project completion. Proper disposal of construction debris must be on land in such a manner that debris cannot enter or cause water quality degradation of State waters. X 8) Concrete leachate must be contained during pouring, such that water quality degradation of adjacent waters does not occur. 9) Excavated materials cannot be used as landfill within 200 feet of the shoreline without prior approval from the shoreline division of the Mason County Planning Department. 10) The applicant acknowledges that this development was sited such that further shore protection measures will not be required for protection of the facility. 1 1 ) Approved per dimensions and setbacks on submitted site-plan. X Attachment for SEPA Continued Question #11 Purpose of re-construction Our family has had our waterfront cabin at Lake Cushman for 25 years. I have fond memories of sailing a small sailboat and fishing throughout the lake when I was a child and young adult. My sisters enjoyed water skiing and swimming. My father used his time at the lake to relax from the pressures of his job. My mother, kept the cabin going at full speed. The bulk head provides: protection for the upland property/ 900 sq. ft. cabin, access to a small free floating dock, and provides a safe place for small children to play near the lake. Now 25 years later, I hope my two sons can enjoy these same pleasures. My sisters have family's of there own who also enjoy the lake. Recreational property provides our children with a place to enjoy and learn about the environment, and even better yet, stay out of trouble. I have seen Lake Cushman change over the years from a rustic out of the way special place to a bursting place that has been discovered. However, the beautiful scenery, quiet mornings and evenings with the family always remain special. During the summer of 1998, a storm exasperated the already structurally damaged bulkhead. There are numerous large cracks throughout the structure, and the front has been pushed out. The loss of the bulkhead and the soil behind it could impact the lake. This, coupled with the potential for loss in property value and use, are chief concerns to our family. It is imperative that the bulkhead be replaced before the water level rises. Although concrete may not be the best substitute for natural shoreline, I believe this project is environmentally sound. It is not practical to construct the bulkhead out of rock, as the bank would be to steep to transport the material. This project is replacing an existing structure in the identical location, and the structure will not encroach any further onto Tacoma City Light Property. Our neighbors to the north have allowed for a small section of natural shoreline. The steps built into our project will allow for some water to break the wave action. Thank you for your consideration in this project Attachment for Sepa question#11 (see attached drawings also) This project is intended to replace an existing bulkhead. The current structure has failed because of: age, lack of sufficient rebar, inadequate drainage within the structure and minor undermining. If the bulkhead is not replaced, it will probably fail and the concrete walls and fill material will fall into the lake which may result in environmental degradation. The new bulkhead will be professionally designed (see attached drawings) with rebar every square foot. The dimensions are 70' long 8" thick I V to the upland. Note: These are the exact dimensions of the existing the structures except for the thickness (6') and lack of wave breaking steps. Two wing walls will go back toward the bank at the location of the steps. At the South end of the structure a wall will be placed inside of the adjoining bulkhead. On the opposite side the wall will join further toward the shoreline. The inset on the neighboring bulkhead allows for natural wave action. The wall will be formed using conventional techniques of plywood form boards. The concrete will be pumped from the drive way to the cabin over 180' slope distance. The concrete will be a single pour to increase the strength. Parts of the old bulkhead will be demolished and placed behind the new structure, the remainder will be cabled up to the cabin Although concrete may not be the best substitute for natural shoreline, I believe this project is environmentally sound. A small excavator or back hoes will be used to dig out the footing and placing the old bulkhead behind the new structure. The machinery will be brought in through State Park land which is at the end of potlatch drive. Note: State Parks has granted us permission to use the old road. It is approximately 1000' from their property to our site. Extreme care will be taken when the machinery is on the beach. The machinery will only access the site once and will probably be there only one day. Minimal disturbance will take place when the machinery is "walked" into the site. Oil absorbent pads will be on the machine in case of an accidental oil spill. The proper authorities will be notified in case of such a spill. Only two trips are required over to the will take place over the beach. I have selected a contractor that has worked expensively with the Dept. of Fish and Wildlife and Mason County on projects near or on the Hood Canal Shoreline.
