ATTENDANCE: Laura Porter, Chairperson Michael D. Gibson, Board Member J. Joanne Hoover, M.D., Health Officer Brad Banner, Health Services Director Sharon Bisser, Nursing Supervisor GUESTS IN ATTENDANCE: Steve Smith, Mason General Hospital; Linda Collie, WWAHEC; Sherri McDonald, WSDOH; Marlyce Hansen, Shelton DSHS. ADMINISTRATION HEALTH PLANNING AHEC/APEX Brad Banner, Health Services Director, introduced Linda Collie of the Western Washington Area Health Education Center, to discuss the AHEC; and Sherri McDonald, Washington Department of Health, to discuss the Assessment Protocol for Excellence in Public Health (APEXPH). He stated he wanted to update the Board on the planning process and decision making process. There will be a policy decision as far as entering into a contract with AHEC. Last year, Mason County didn't receive a Rural Health Grant. However, money was received to help reapply for funds. Nancy Field went to work for Mason County. Basically, the original conception of the Rural Health Grant is that it is a one shot grant. One set amount of money used to catch people that fell between the cracks of the medical system. The people that were making enough money, but were not eligible for medicaid. When Ms. Field started working on the grant she saw a more fundamental need is health planning among health agencies. She felt the medical programs in place needed to pull together and have a common strategy. The Health Department is also involved in a different process of internal assessment. The two are fundamentally similar. The County is working to try and combine the two. Linda Collie,Western Washington AHEC,explained that they have a project done in the WAMI (Washington, Alaska, Montana & Idaho) area through the University of Washington School of Medicine. It is a strategic planning process. They do a community survey of 1,000 people to understand the perceptions of the community, as far as health care is concerned. Then a needs assessment is done with key informants and other groups. They usually do an organization and financial review of the hospital. In this case, they are proposing to do a joint venture between health department and the hospital. There will be an interagency organizational and financial review; scope of services to determine what health care is provided in the community. They will determine what is feasible for a community of this size with the providers. It could be determined what other services might be offered. MASON COUNTY BOARD OF HEALTH SEPTEMBER 9, 1991 - PAGE 2 The major thrust of this whole process is to develop a community counsel. That group does a formal strategic planning for the community or County. They are looking at a broad based community strategic planning group to go on and implement the plans that the committee comes up with. Board Member Gibson asked if the University of Washington would do the strategic planning. Ms. Collie responded that since Nancy Field has already worked in some capacity in the community, they will try and participate here in this. The funding comes from the U of W School of Medicine. She explained that AHECs were mandated by Congress 20 years ago. They do a variety of activities to increase access to rural health care. Board Member Gibson asked if there is a length of time for this program. Ms. Collie stated that tentatively they were looking at a year, however, it might not take that long. They are hoping that the State might fund through the Rural Health Development Grant program. The State can't pay for anything that is already done. There won't be funds available until February 1, 1992. The County should be in the strategic planning by about that time and there should be implementation in less than a year. Sherry McDonald, Senior PHN with Washington Department of Health, liaison between State Department of Health and all Local health departments in the State. She noted that Mr. Banner has expressed interest in using this tool ("APEX"). She explained that it is a tool. It is a work book, Assessment Protocol for Excellence in Public Health. It has been developed over several years and started in 1987. It has been tested in about 50 states across the nation in health departments that are smaller and larger than Mason County. The reason the State Department of Health is involved at this point is that the Center for Disease Control and NACHO are working on helping local health departments across the state. Washington is very interested in using this tool so that the capacity of public health in Washington State could be assessed. The first part is meant to look at the internal organizational capacity of the Health Department. It will determine if there is a capacity to provide services where the County is located. The second part is a community process. The APEX tool is meant to be a data driven, community driven process. It is very complimentary to AHEC. AHEC will provide data from Department of Health. They look at data in different perspectives. The third part of the APEX tool is meant to look at planning as an issue. Ms. Collie stated they are hoping Mason County can be used as a model in all of the WAMI areas. Marlyce Hansen, Administrator of Department of Social & Health Services office in Shelton. She stated she is in support of this project. One of their concerns is the health care needs of the poor. Currently, they administer 54 - 57 separate medicaid programs for residents in Mason County. The number and complexity has increased drastically, in particular as it relates to children. Pediatric care is hard to find and they feel the survey will be helpful in identifying further gaps in the community that need to be addressed. MASON COUNTY BOARD OF HEALTH SEPTEMBER 9, 1991 - PAGE 3 Steve Smith, Superintendent of Mason County Hospital District #1; and Administrator of Mason General asked that the Board help provide up to $50,000 for the survey. He stated that is what the survey is worth from the University of Washington. The County can get this for $15,000. It will cost us $15,000 to contract with AHEC for the survey, the implementation and strategic planing. He expressed that he hopes that the State will come up with $7,500 for the Department of Rural Health Grant Development funding. There is not commitment,however, there has been a commitment from timber distressed dollars per Tammy Hupp in the amount of $2,500. Mason County Hospital District #1 has voted for up to $2,500 towards this program. They will be asking the City of Shelton for $2,500. That leaves the County and the County Health Department of $2,500 for a project that would be worth up to $50,000. He noted that there is a steering committee established. Brad Banner and Sharon Bisser sit on the committee. Also, there is a physician representation and there will be a private business representative. The main objective is to identify high priority health needs of Mason County residents through planning and address health needs with community cooperation. He felt that the best thing he could see is to get a dysfunctional system into a better cooperative effort in the County. Chairperson Porter stated that the Health Board could make a recommendation to the Board of County Commissioners for allocation of the funds. Board Member Gibson/Porter moved and seconded to go on record supporting the project. Motion carried unanimously. G-aye; H-absent; P-aye. Board Member Gibson noted that there won't be a final answer for funding until December when the budget is approved. Mr. Smith asked if it approves authority for someone to enter into a contract with AHEC. Board Member Gibson stated he has a problem making a commitment out of the 1992 budget. That will be up to the Board of Commissioners. Chairperson Porter concurred that she would like to see a contract signed for current years monies. The Board will set up a briefing with Brad Banner to discuss this. Mr. Banner to provide the Board with a copy of the MOU with AHEC for briefing. 1992 CONSOLIDATED CONTRACT Brad Banner,Health Services Director,presented the Board with the authorized signature sheet for the Consolidated Contracts. MASON COUNTY BOARD OF HEALTH SEPTEMBER 9, 1991 - PAGE 4 Board Member Gibson/Porter moved and seconded that the Health Services Director be authorized to sign the 1992 Consolidated Services contract. Motion carried unanimously. G- aye; H-absent; P-aye. Board Member Gibson/Porter moved and seconded to amend the motion to recommend to the Board of County Commissioners. Amendment carried unanimously. G-aye; H-absent; P-aye. DEPARTMENT ACCOUNTING SYSTEM The Board was updated on the Health Services'accounting system. Mr.Banner stated that John Church has been working on the sytscm off and on for a month. He set up the system and had 99.9% done before the reorganization came along. Mr. Banner stated that since the reorganization he spoke with Eva Hobbs the Public Works Accountant and was impressed with her work. He stressed that the advantage of going with Public Works is that the accounts would be supervised by an accountant. He stated that he could foresee potential problems when there are program changes and when it comes to trouble shooting if we don't have accountant supervision. Public Works has similarities in accountability for reports. Board Member Gibson stated he would like to look at it from the three departmental aspect. Chairperson Porter asked that they discuss this with Marley Young and then come back to the Board in a briefing. PERSONAL HEALTH HEALTH OFFICER Dr. Hoover informed the Board that Terry Johnson, HIV Coordinator, sent out invitations for an update information session on HIV AIDS to medical providers in the County. Dr. Bob Woods, Director of AIDS control program for King County, will be present as well as other knowledgeable guest speakers. The meeting will be at Orient Express. There will be 106 people. Sharon Bisser,Nursing Supervisor,stated North Mason called and expressed their appreciation for the special clinics offered and services provided. INFECTIOUS DISEASE POLICY Brad Banner noted that they are continuing to work on the policy without changing the substance, but to help clarify it more. The most recent revision has been reviewed by Dr. Hoover and Chairperson Porter. He stated that he wanted to put both comments together in one draft and present at the next Board of health. He recommended one more month to finalize the policy. Board Member Gibson/Porter moved and seconded to delay implementation of the Infectious Disease Policy until 8 a.m. October 8, 1991. Motion carried unanimously. G-aye; H-absent; P- aye. The Board directed the Health Services Department to send letters to all'department heads stating that it has been continued to October. MASON COUNTY BOARD OF HEALTH SEPTEMBER 9, 1991 - PAGE 5 ENVIRONMENTAL HEALTH WATER ADEQUACY ORDINANCE The Board was informed that this is on hold at present. Erik Fairchild, Planning Coordinator, and Robin Munro, Growth Management Planner, have to go over it a little bit more. Section 52 impacts the Planning Department in that when you plat you have to have adequate water. He is looking at alternatives.
