Dear Mr Salzer:
CONREX Inc., acting as a representative for The Washington State Department of Fish
and Wildlife, has developed the following work plan for the decommissioning,removal and
disposal of one (1) underground storage tank (UST) system and the installation of one (1) above
ground storage tank (AST) system at the Washington State Department of Fish and Wildlife
(WSFW), Hood Canal Hatchery facility, located at N 24350 Highway 101, in the City of
Hoodsport, Mason County, Washington.
The UST at the site is currently utilized for the storage of diesel fuel used in the operation of the
facilities emergency backup generator. This UST is being removed from service in accordance
with the requirements of the United States Environmental Protection Agency's (EPA)Federal
Underground Storage Tank Program of the Resource Conservation and Recovery Act (RCRA)
of 1976 and the requirements of State of Washington's Model Toxics Control Act (MTCA),
Chapter 173-360 WAC.
SCOPE OF WORK - UST DECOMMISSIONING
The following work plan was developed for decommission one (1) 1200 gallon capacity
UST at N 24350 Highway 101, in the City of Hoodsport, Mason County,Washington. Prior to
commencing work at the project site, an underground utilities service will be notified and the

location of all buried utility conduits at the property will be marked.
Prior to decommissioning the UST, a temporary above ground storage tank system will be
installed in order to maintain operational capabilities of the generator in the event of a power
outage. Upon completion of the UST removal, an AST system will be installed(Refer to Scope
of Work- AST Installation below) to provide fuel storage for the generator(s).
The UST decommissioning will include obtaining all necessary permits,removing a portion of
the asphalt driveway covering the UST and excavating the soil surrounding the tank. All
excavated soils will be stockpiled onto visqueen plastic and covered at the end of each day. Any
residual product remaining in the tank will be purged from the UST and transferred to the
temporary tank or disposed of at an appropriate treatment storage and disposal (TSD) facility.
After the UST has been purged, cleaned, and rinsed, the tank will be inerted and monitored with
an OZ/LEL meter to assure that appropriate levels are obtained prior to removal. The remaining
soil will be excavated surrounding the tank so that it can be removed from the ground.
Prior to removal, the tank atmosphere will again be monitored to assure that appropriate levels
are maintained. After the atmosphere within the tank has been inerted to a safe level (as per the
American Petroleum Institute Recommended Practice 1604, "Closure of Underground Petroleum
Storage Tanks"), the UST will be removed from the excavation.
Upon removal, the tank integrity will be visually inspected for the presence of holes or leaking
seams. The UST will be properly labeled and subsequently transported to an appropriate facility
for disposal or to be recycled. The UST excavation will then be inspected for visible and
olfactory evidence of petroleum hydrocarbon contamination. The product and vent lines will
either be cut and capped at the limit of the excavation or removed from the excavation
depending on accessibility.
Soil samples will be collected from the UST excavation subsequent to the tank removal.
Additionally, soil samples will be collected from the excavated soil stockpile for chemical
analysis. We do not anticipate that near surface groundwater will be encountered during the
excavation of the UST. However, if encountered, groundwater samples will also be collected
from the UST excavation. All soil and/or groundwater samples will be collected in accordance
with Washington State Department of Ecology's (WDOE)recommended sampling protocols.
The samples will be stored on-site in a cooler with ice until they can be transported under chain-
of-custody to an EPA and WDOE certified laboratory for chemical analysis. Each sample will
be analyzed for the presence of petroleum hydrocarbon contaminants in the diesel range using
EPA approved test method 8015 modified for Diesel; WTPH-D.
Upon receiving the analytical results verifying that soil and/or groundwater contamination is not
present at the site above the established cleanup levels, the excavation will then be backfilled

with the overburden soil and clean imported fill material (pit run). In the event that soil and/or
groundwater samples reveal the presence of contamination above regulatory action levels, site
cleanup actions will be conducted at the facility in accordance with WAC 173-360-375. All
contaminated soils will be temporarily stockpiled at the site on visqueen plastic and covered at
the end of each working day. All contaminated soil material will be properly disposed of at an
approved TSD facility.
A report will be submitted within four weeks of completing the field activities. This report will
include a discussion of the site conditions, a description of the field activities, the findings of the
investigation, and recommendations for further assessment or remedial activities should they be
necessary. The report will include a copy of the WDOE 30 Day Notice, the UST Site
Check/Site Assessment Checklist and the UST Closure and Site Assessment Notice, a copy of
any UST removal permit(s), laboratory analysis results, copies of the UST disposal certificate(s),
and soil disposal certificate(s) when applicable. The report will also include a brief description
of the UST decommissioning, and relevant soils/geologic information, and site maps showing
the locations of work preformed and sample collection locations.
SCOPE OF WORK - AST INSTALLATION
The following work plan was developed for installation of one (1) 250 gallon capacity
above ground storage tank(AST) system at the aforementioned site. The AST system will be
utilized for the storage of diesel fuel used to operate the emergency backup generator at the
hatchery facility.
Prior to installation of the AST system, a concrete slab will be constructed upon which the AST
will be placed. Specifications for the foundation are included in the site drawings. The new
concrete slab will cover an area of approximately 53 square feet. The AST will be placed onto
the slab and secured in place. A total of 5 bollards will be installed around the perimeter of the
foundation pad (Refer to specification drawings).
The associated AST piping will then be installed to connect the AST system to the emergency
generator. The specifications for the piping are shown on the site drawings.
When completed, the AST system will be connected to the emergency generator system and the
temporary tank will then be taken off-line and removed from the site. Any fuel remaining in the
temporary tank will be transferred to the new AST system or disposed of at an approved TSD
facility.

Underground Fuel Piping System Specifications
The underground fuel system shall be comprised of components of an inte-
grated design which shall afford the greatest reasonable degree of survivability
and performance, without environmental contamination, in the event of seismic
activity.
The primary piping shall be continuous and have no buried or inaccessible con-
nections or joints. All primary piping connections shall either be above grade or
accessible inside subsurface chambers.
The primary piping shall:
f 1 . Meet the minimum standards of Underwriters Laboratories Inc. ® "QXLT Nonme-
tallic Piping, Flammable Liquid, Underground Pipe intended for the underground
transfer of petroleum products at temperatures between -40C and 49C when
installed" testing and be so listed.
2. Be flexible and capable of a minimum bend radius of 6 inches without kinking,
cracking or breaking.
3. Have a minimum burst strength rated at 5 times the anticipated working pres-
sure developed by the specified pressure pump.
4. Have sufficient internal hoop strength so as to not collapse under the antici-
pated vacuum produced by the specified suction pump..
5. Have metal swivel fittings affixed to the ends in the same machine crimped
manner as when piping was tested by UL & ULC.

Secondary Piping:
The Secondary Piping shall:
1 . Meet the minimum standards of Underwriters' Laboratories of Canada, testing
for "Nonmetallic Underground Secondary Containment Piping, for Petroleum
Products", and be so listed.
2. Be flexible and capable of a minimum bend radius of 24 inches without kinking,
cracking or breaking.
3. Be of sufficient strength to withstand "H-20" burial loads in accordance with
AASHTO M294.
4. Be of sufficient size and design so as to allow the containment of up to four
separate primary pipes of 1 /2" and/or 3/4" size and also allow the removal
and/or replacement of the primary piping without excavation or compromise of
its integrity.
Other System Components
Primary & Secondary piping terminations:
Termination into subsurface containment chambers shall be effected via watertight
bulkheads which shall not expose any metallic components the backfill or native
soil.

Terminations above grade or within buildings shall be effected via assemblies
which shall be designed so as to be rigidly located through a vertical wall or
horizontal paving, while maintaining a watertight seal and the flexible termina-
tion of both primary and secondary piping.
System Testing
System shall be designed so as to allow the independent, temporary, pressure
testing of all primary and secondary piping simultaneously, whether at the time
of installation or on future occasions.
Testing upon installation.
1 . Primary piping shall be capable of air pressure testing to a minimum of 60 psi to
a maximum of 250 psi.
2. Secondary piping shall be capable of air pressure testing to a minimum of 3 psi
to a maximum of 5 psi.
3. All piping and conduit bulkhead seals into subsurface chambers shall be tested
for watertightness.
System Monitoring Capability
System shall be designed so that all piping may be sloped to a low point or to a
subsurface chamber in which a sensor may be placed to detect leakage from
the primary piping.
