Two (2) copies of this form are to be submitted to the County prior to the issuance of a building permit. If changes are made as to who will perform the special inspections a new form shall be submitted and turned into the Building Permits&Inspections Division for approval. STATEMENT OF SPECIAL INSPECTIONS: In addition to this Agreement, a "Statement of Special Inspections" shall be, provided per IBC 1704.3. This Statement shall be made as part of the approved plans, and be placed in a conspicuous location,such as the first page of the construction plans or the first page of the structural sheets. SPECIAL INSPECTORS: All special inspectors shall be approved by the Building Division prior to performing any duties. The special inspector shall provide proof of certification as a special inspector for each inspection item and be licensed by the State of Washington. SPECIAL INSPECTION REPORTS: Special inspection reports are to meet the requirements of IBC 1704.2.4. Copies of each report are to be sent to the address listed in the letterhead noting the Project Address and Permit Number. A final report • shall be submitted stating that all special inspection and structural testing items were completed and are in conformance with the approved design drawings and specifications. Items not in conformance, unresolved items,or any discrepancies in inspection coverage(i.e. missing inspections, periodic inspections when continuous was required, etc.)shall be specifically itemized in the final report. Gerth, P.E., Chief Engineer WDFW McKernan Hatchery Bridge Special Inspection Report December 15, 2016 . The Mckernan Hatchery bridge project consists of replacing culverts with a concrete precast, prestressed,voided slab bridge and adding a fish screen structure with grated walkway. The bridge was purchased from Concrete Technology Corporation. CTC is certified by WSDOT and B4/C4 certified by Precast/Prestressed Concrete Institute (PCI). All concrete work for the bridge deck was certified and inspected by CTC staff. CTC certification was provided with the building permit application. A fully designed and constructed bridge was purchased by Washington Department of Fish and Wildlife (WDFW.) WDFW is the owner and the design engineer for this project. WDFW hired a contractor to install the bridge and the walkway. Boettcher and Sons Construction. The contractor was responsible for the pre-cast foundations and assembling the aluminum walkway. WDFW staff was on-site performing water quality testing and construction inspection throughout the project. As required,continuous inspection was performed by WDFW construction inspection staff, on the pre-cast concrete work performed by the contractor including: • Rebar inspection • Formwork • Warm weather curing protocol • Concrete pouring Concrete cylinders were taken by WDFW construction inspection staff, cured appropriately and sent to the WSDOT testing lab in Lacey, WA for 3, 7, and 28-day compressive strength. Appendix A has the WSDOT test results. WDFW inspection staff consists of Construction Project Coordinator 1's and 2's with a minimum of 2 years construction project management and inspection experience. Inspectors are supervised and daily inspection reports peer reviewed. Inspection staff in overseen by a professional engineer who reviews the photos,visits the site, and reviews the reports. Any discrepancies or questions are identified and addressed by the professional engineer. All inspectors are approved by the WDFW Chief Engineer, Engineer of Record. Staff for this project include: • Chris Alston CPC1- Inspector • Joshua Brown, PE - Inspector • Casey Davidson CPC2—Bridge Inspector • Kristen Kuykendall, PE—Project Engineer • Glenn Gerth, PE—Chief Engineer, Engineer of Record Daily inspection reports are generated documenting the quality and quantity of work completed. The Inspection Daily Reports(IDR's)for this project are included in Appendix B. • Secondary to the bridge is the fish screen support structure with walkway. The foundation slab for this structure was poured with the bridge abutments. It meets all strength specified by the engineer of record. The walkway was anchored to the slab with stainless steel bolts. Identified in the stamped drawings the bolts were installed as designed. The model of the bolt was changed with the approval of the project engineer and the engineer of record. The strength of the bolt was not changed and installation was inspected by WDFW Staff. All work was approved by the project engineer. The walkway is gated and only accessible to trained WDFW hatchery and construction staff. Appendix C has the approved anchor bolt cut sheet. Appendix D is a collection of inspection photos,verifying proper construction technique, careful inspection,and adequacy of the project. As the owner, engineer, and operator of the hatchery and the bridge project, all aspects of fabrication and installation are approved as being to the design standard. Once rigid screens are in place,de- Mechanical HVAC fishing begins. Plumbers Painters . Crew ties steel abutment cages up in the meantime and completes. I checked all dimensions Electrical WorkersConcrete Finishers for correct clearance/dimensions. All appears to be in order according to specifications and Roofers drawings. Forms to be constructed in the remainder of this week. Concrete to be placed on Equipment erators 1 Monday 22 August,tentatively. Time to be determined later. Owner Operator To date,no concrete submittal has been received from contractor. I give a call to Mr. Boettcher and remind him to submit concrete submittal prior to pouring the abutments. Contractor acknowledges and agrees to have the submittal to Kristen or myself by Friday 19 August. Total Workers 3 Some discussion between contractor,Jorge Villareal and I regarding dates on HPA for in- water work. No large woody debris has been placed as yet,and contractor has only two Equipment No. weeks to complete in-water tasks. Contractor asks about possibility of extending HPA work Pick-up Truck 1 window. After discussion with Kristen,it is agreed CAMP will not seek an HPA time Excavator 2 extension on the contractor's behalf. I let contractor know by phone that WDFW expects 4"Pump Diesel 1 him to hold to the schedule and install all in-water work by 31 August as the permit dictates. He acknowledges and agrees to work to that goal. Equipment trailer 1 Discussion between contractor and I regarding use of neighbor's driveway to facilitate bridge delivery truck. I explain that this will be up to him and bridge delivery company. I supply neighbor's phone number and name to him. While on site contractor's excavator arrives on site. Appears to be a 135 series machine. (3) Carpenter 1 separate panels constructed with interlocking"knock-outs"built in. Check all panels for Laborers 2 overall dimensions and all appears correct at 26' 1-'/2"x7'x 12"deep. Panels appear square. Iron Workers Mechanical HVAC Laborers preparing flat level surface to cast both in-stream panels and abutments. Laborers Plumbers taking care to make the surface clear of gravel and compacting using a plate compactor. Painters Electrical Workers Surface has been covered by construction fabric and wetted slightly. Concrete Finishers Roofers Likewise,I've checked abutment forms for true and correct dimensions and they appear to Equipment erators 1 check out well. All reinforcing for both the panels and abutments appear correct in Owner Operator dimension and clearances from top of subgrade,sides of form and top of concrete surface. All bar intersections are true 90' angles and firmly hand tied with wire. No dobies are used in this pour. All cages and mats are hung from temporary 2x4 trusses and wire. This makes a clean looking install with no opportunity for cracking at dobies. On the East end of project,I've checked progress on the LWD jams. On the right bank side, Total Workers 4 just DS of the weir,only 5 out of 6 log/wads have been installed. Install is not complete as the contractor has yet to install the Manta Ray anchors. No woody debris has been installed Equipment No. on the Left bank side yet. Contractor understands from my conversation with him this Pick-up Truck 2 morning,and reviewing the prints together that he will still be required to install all small Equipment Trailer 2 root wads as depicted in prints and anchor all logs using(2)minimum anchors per log. That 12"Pump Diesel 1 makes a total of 24 anchors which he acknowledged. Contractor has silt boom deployed in water as called out by permits and specifications. Contractor also knows he is required to 4"Pump Diesel 1 submit silt and turbidity testing,(as per acknowledging Kristen Kuykendall's verbal Excavator 2 reminder and discussion)but declined to produce a log of testing while he was constructing in-water during the past weekend. All logs installed to this point appear secure and as per prints and expectations. Nichols Trucking,contractor to bring bridge to site,was on site today with truck to make a "dry run"test. Driver states that he had no trouble getting his rig into the site,although it is a bit snug around corners. Driver stated he could see no reason why bridge couldn't be delivered to site on his truck. Inspector: Engineer Signature Signature Date for bridge delivery remains the same as Friday,26 August. Contractor is coordinating crane to be on site at same time as bridge arrival so that crane can make one pick from truck to abutments,pending acceptable concrete cylinder break test from MATS lab. Concrete was placed in forms today starting at 1:OOPM. After 1st truck arrived,I waited for approx. '/2 CY to be discharged to clean out area and took my sample. Conrete temp 77' and tested on site at 5.5 slump. Truck ticket states that concrete was mixed to 4"slump. I made three(3)sets of two cylinders. Two(2)each for 7, 14,and 28 day break. Cylinders were placed in WDFW's picknic cooler in approx. 8"depth tepid water. Cooler was left on grassy level surface in shade beneath large Oak tree in residence yard. Cooler will be transported to MATS lab on following day. Contractor placed concrete in lifts in each form,approx. 8"lifts and vibrated gently between lifts. Concrete to be troweled off and given a light broom finish. Concrete placement was finished approx.2:45 PM. Two trucks total for 13CY placed. Trade No. Project Manager Arrive on site with Chris Alston and Josh Brown 9:30AM Construction Manager Superintendent 1 Contractor on site and currently working on engineered log jams on right bank scour hole in Carpenter work area 2. Laborers 1 Iron Workers Current log jam is coming together and appears to be in line with prints and WDFW Mechanical HVAC expectations. No anchors have been installed yet,but are on site for both log jam locations. Plumbers I verified that the contractor has the correct number of anchors to fulfill requirements and Painters had brief conversation(again to reinforce W Electrical Workers DFW requirements of minimum 2 anchors per Concrete Finishers log). I asked Caleb when he anticipates installing the anchors and explain that WDFW want Roofers to be on site during the install,and Caleb only tells me that"it will be early in the next Equipment erators 1 week." I instruct Caleb to provide an exact date by end of work day on Friday 26,and he Owner Operator agrees. Log jam on left bank has not been started yet,but silt boom is in place per permit requirements. Only four logs on site for the left bank jam,but Caleb confirms he has the last two logs set to arrive late this week. While onsite,I witnessed Caleb and an employee assistant walk downstream to take water samples of turbidity. I did not verify quality of water or sampling technique,but I did verbally reinforce to Caleb the importance of following through with the water sampling requirements. Total Workers 3 I had a brief conversation with Caleb regarding date of bridge arrival on site. Current date Equipment No. of arrival was set for Friday 26,the date has yet again been pushed back to next week. At Pick-up Truck 1 this time,Caleb declined to give an exact date,rather tells me that the bridge will be on site Equipment Trailer 1 "Thursday or Friday". I instruct Caleb to call or email with an exact date and approximate 4"Pump Diesel 1 time by end of work day on Friday 26,and he agreed to follow through. 12"Pump Diesel l About half of the ecology blocks for lining the channel are on site. Caleb tells me the Excavator 2 remainder of blocks are on their way and should be on site by end of day today. Each was pulled up about 8"to set them.The ground appeared very solid when the anchors were driven and they used a 10' steel rod to get the anchors to the correct depth of 6'.The silt boom on both banks has been removed and work is complete at site 2. There is a space on the block wall(between blocks 2 and 3)on the right bank that is about 1- Total Workers 4 2"wide.Caleb was adamant that the blocks are sitting on the locking guides on the block below. Jack Hammer 1 Cutoff Saw 1 Dave offsite around 1:15pm. 4" um 1 12"pump 1 At 1:30pm they started the 4"pump to dewater the work area and they began excavating behind the block wall on the right bank.They excavated down—3' and placed—1 Ocuyd of quarry spalls in the excavated hole(3'x4')which was the length of the abutment.This was compacted with the smaller excavator. Conversations 10:45am Casey and Ispoke to Caleb.He told him to add a second row of blocks under upstream blocks on the right bank,except where the electrical wire is located.Caleb is very unsure of the bridge installation.He made several comments on the amount of water and the difficulty it is causing.Casey urged him to get supersacks onsite to delineate the work area in order to remove the culverts. 12pm Kristen,Casey and I spoke to Caleb about the plan for the site.We suggested using a full coffer dam upstream and downstream and pumping the water using the 12"pump that he has onsite to bypass the water.Defishing will happen on Friday this week and Caleb agreed to that.He was reluctant about placing another block under the upstream end of the block wall on the right bank.He agreed to add 1 block at the corner after some discussion about the power line location.He also had reservations about the spalls behind the block wall but got started after Kristen pushed him to get moving. 3:30pm Kristen,Casey and I spoke to Caleb and Casey gave him a suggested plan for the next few days(until Tuesday)and he was ok with it.The plan laid out getting the left bank wall in place and placing the quarry spalls and ballast stone on Thursday/Friday as well as defishing on Friday.Then excavate the center area on Saturday,remove the culverts and establish a gravity bypass.Then start placing the walkway on Tuesday.Kristen suggested he put a screen downstream with the supersack coffer dam for the gravity bypass so he doesn't need to defish again next week as he won't want to run his pump all weekend. Carpenters have Plumbers sacked in most visible portions of abutments and slabs,but more work will need to be done Painters Electrical Workers on keyways to ensure a tight positive fitment once installed. Concrete Finishers Roofers At East end of project,crew is currently working to complete the anchor install on logjams- Equipment erators 1 Right bank jam has been installed and all cable wrapping is in place. Although I did not Owner Operator witness the install of anchors at this jam,Caleb Boettcher assures me that all correct depth of install were met. Wraps appear correct with 2 wraps per installed anchor. Although all anchors received the minimum 2 wraps required,method of install actually ties multiple logs together. This should actually make for slightly stronger bind overall. I did not witness the crew"set"the anchors. I inquire with Caleb and he confirmed that the anchors were not set. After witnessing the install of the Left bank jam anchors,I am comfortable with the quality Total Workers 4 of install as the anchors are driving in slow and firm. I visually confirmed that correct depth was achieved based on length of driving rod. Crew using 10'rod,driving to depth that Equipment No. leaves approx.3' of rod above stream bed. This will allow for approx. 12"of pull back to Pick-up Truck 2 ensure correct set of anchors. After all anchors were installed,crew connected excavator to Dump Truck 1 each anchor and pulled back 9-12"to firmly tip the anchors under the soil,setting them. I Excavator 2 then watched the crew wrap a handful of logs and strap the stainless cables via mechanical clamps. After this work was completed,the crew pulled in water silt booms from each 12"Pump Diesel 1 portion of in-stream work. 4"Pump Diesel i Spoke at length with Caleb Boettcher regarding the excavation and install of abutment sub grade. I explained to Caleb that his crew will need to pick up the pace of work to minimize the delay of bridge install. I made Caleb aware that today is the last day for permitted in- water work and he acknowledge. Caleb expressed concern with amount of water present in abutment excavation and poor quality of soils. I stated that WDFW is requiring him to continue working to install abutments and that the project will not stall as is his suggestion. At this time the contractor has approx. IOCY of spalls on site for install beneath abutments. Inspector: Engineer Signature Signature The amount appears inadequate and I ask Caleb to be sure he will have enough spalls on site to complete the abutment install. Likewise,there are not enough sand bag super sacks on , site to intall coffer dam to delineate the culvert work area from open stream. I strongly suggested to Caleb that he evaluate the number of bags he will need to complete the coffer dam and to consider getting more super sacks on site. He acknowledged and agreed to work on that. Currently,rigid screen were breached upstream of work area for unknown reason. I instructed Mr.Boettcher to re-install the rigid screens and keep them in place as depicted by the project prints. Approx. 12:00 Noon,Kristen K.arrives on site to review progress. After a quick look around,Josh,Kristen and I confer with Caleb and review his approach on installing the abutments. Contractor installed 4"diesel pump into abutment pit and began to dewater to upland infiltration site. Operator began excavation of unsuitable materials and began to install quarry spalls in pit. Although unsuitable,material does not appear to be as poor in quality or great in quantity as described by contractor. After spalls were installed,the subgrade really appeared to firm up and provide a stable base for install of ballast as called out by prints. Toward end of my site visit today,the contractor's truck returned and began to install ballast material over spalls on South abutment. Much discussion was given by Kristen,Josh,Mr.Boettcher and I. Mostly regarding upcoming tasks and schedule for the next few working days as well as install and use of 12" bypass pump. At this time,no parts of pump have been assembled. WDFW collectively offered suggestions to Caleb on how he might consider installing and routing pump lines. Further,we offered suggestions on how he might proceed with excavation of fill material and removal of culverts while correctly bypassing Weaver Creek through the excavation. Mr.Boettcher brings our attention to several challenges,but every point raised was thoroughly discussed. Prior to leaving site,Kristen,Caleb and I completed a correction items list and outline a list of tasks by each upcoming work day that WDFW will expect Mr. Boettcher to meet and complete.
