Dear Applicant,
This letter is to inform you that the above reference design for a Group B public water system has been reviewed and is
hereby APPROVED with the following conditions. Full approval of the water system for use will require the following
items:
1. Construction Report for Public Water System Projects within sixty days of construction
completion.
2. Installation Inspection approval(please call to make an appointment)
After Installation approval an updated WFI form will be submitted to Washington State Department of Health, where the
water system will receive its unique ID number. If you have any further questions let us know.

1 NOTICE:All proposed connections to new wells are subject to water adequacy
requirements at time of building permit per Mason County Title 6.68 Water usage
restrictions and additional fees may apply to all new wells drilled after January 19th, 2018
per ESSB 6091.
