Addendum 1 to Geotechnical Report
North Fork Skokomish Salmon Hatchery and Saltwater Park Sockeye Hatchery
Hoodsport,Washington
Dear Mr. Druffel:
At your request, we prepared this addendum to the Geotechnical Report pr.-pared for the project
referenced above. This letter provides additional information requested by Mason County to
assist with their review of the project plans.
Background
Tacoma Power has designed new fish hatchery facilities to be constructed a:their North Fork site
near Cushman Darn No. 2. Comforth Consultants prepared a geotechnical report for the project
in December 2013. As part of the plans review process, Mason County requested additional
information to address specific items in Mason County Ordinance 17.0 1.10 D—Landslide Hazard
Areas.
Responses to County Review Comments
The following items provide additional information and/or clarification to address the County's
review comments and questions. The comments are arranged and referenced as listed on the
County form titled"Submittal Checklist For a Geotechnical Report."
1. Item #4. The County defines the slope on the east side of the North Fork site as a landslide
hazard due to the presence of slopes steeper than 40 percent. The attached Figure I shows the
proposed hatchery structures and the slope that is defined as a land Tide hazard. County
Ordinance 17.01.100.D.6.a requires a 50-foot buffer of undisturbed, natural vegetation
around an area defined as a landslide hazard. Natural vegetation can lower the potential for
shallow slope instability and erosion, particularly near the crest of a sslope. The site of the
proposed hatchery is currently a gravel parking lot at the toe of the slope. The parking lot

extends to the toe of the slope. There is no native vegetation beyond tie toe of the slope, as
shown in the picture below. From a geotechnical standpoint, there is no benefit from a
vegetative buffer at this site. For this reason, we have shown our rec mmended buffer line
coincident with the toe of the slope on Figure 1.

2. Item#7d. This comment relates to recommended vegetative buffers from the area defined as
a landslide hazard. As discussed under Item #4, there is no geotechnical benefit from a
vegetative buffer at this site, so no buffer is recommended.
3. Item #7e. This comment relates to recommended building setbacks fron the area defined as
a landslide hazard, shoreline bluffs, and the tops of other slopes on the property. The North
Fork Hatchery site is not near the top of any slope. Stability analyses summarized in the
geotechnical report resulted in calculated factors-of-safety for slope stability of 1.50 and 1.13
under static and seismic conditions, respectively. Based on these results, the likelihood of a
large-scale slope failure impacting the development is low. In our opinion, no setback is
required.Figure 1 shows the recommended setback line coincident with the toe of the slope.
4. Item #10. This comment is related to on-site and off-site impa is of the proposed
development on the area defined as a landslide hazard. In our opinion the proposed
development has no on-site or off-site impacts on the stability of the slope.
5. Item #11. This comment is related to vegetative buffers for the propo ed development. As
discussed in Item#4 above,vegetative buffers provide no geotechnical benefit at this site.

We have a construction stormwater permit through Ecology. And as part of that w have a construction
SWPPP drafted that we are having the General Contractor update and implement. Do you want to see those
documents since they are the primary source of answers for the checklist questions I've been confused with
this checklist since it is exclusively for construction stormwater which in our case the State has jurisdiction, and
our submittal documents to Mason County are for the permanent stormwater perrr it.

In accordance with Washington Administrative Code (WAC) 197-11-340, a copy of the
Determination of Nonsignificance (DNS) for the project described below is transmitted.
Applicant: Tacoma Public Utilities/Tacoma Power
Proposal: The Tacoma Public Utility Board proposes to approve contract for the
construction of the North Fork Skokomish Salmon Hatchery. The project is
required mitigation for the Cushman Hydroelectric Project, FERC No. 460.
The Federal Energy Commission, Final Environmental Impact Statement,
FERC/EIS-0095F dated Nov 1996, SEPA Adoption of FERC FEIS for
Cushman Project dated September 2009, National Marine Fisheries
Biological Opinion supplement dated March 31, 2010, US Fish and Wildlife
Biological Opinion supplement dated April 22, 2010, and FERC Order
Amending License issued July 15, 2010, are the environmental documents
covering the need for salmon production but did not specifically cover the
impacts of hatchery location, construction and operation for which this DNS is
issued.
Location: The hatchery address is 30 N. Dow Creek Drive, Hoodsport, WA 98548.
The hatchery is on Lot 126, Plat of Lake Cushman No. 9. The water supply
and waste pipelines cross Lot 128, Plat of Lake Cushman No. 9 (public
boat launch) and continue into unplatted Lake Kokanee.

The City of Tacoma, the lead agency for this proposal, has determined that the requirements for
environmental analysis, protection, and mitigation measures have been adequately addressed in
the development regulations and comprehensive plan adopted under Chapter 36.70A RCW, and
in other applicable local, state, or federal laws or rules, as provided by RCW 43.21 C.240 and
WAC 198-11-158. The City will not require any additional mitigation measures under SEPA.
Additionally, the City of Tacoma has determined that this project does not have a probable
significant adverse impact on the environment. The proposal will have no significant adverse
environmental impacts to fish and wildlife, water, noise, transportation, air quality, environmental
health, public services and utilities, or land and shoreline use. An environmental impact
statement (EIS) is not required under RCW 43.21 C.030(2). This decision was made after
review of a completed environmental checklist and other information on f le with the lead
agency. This information is available to the public upon request.

Construct the North Fork Skokomish Salmon Hatchery for the production of Col,o, Steelhead and Spring
Chinook salmon to restore fish populations in the North Fork Skokomish. This atchery provides

administrative space for hatchery staff, incubation and rearing facilities for juveniles, holding facilities for
adult salmon returns, covered parking for hatchery vehicles, water supply facilities and effluent water
treatment facilities.
12. Location of the proposal. Give sufficient information for a person to understand the precise location
of your proposed project, including a street address, if any, and section, township, and range, if known. If
a proposal would occur over a range of area, provide the range or boundaries of the site(s). Provide a
legal description, site plan, vicinity map, and topographic map, if reasonably available. While you should
submit any plans required by the agency, you are not required to duplicate maps or detailed plans
submitted with any permit applications related to this checklist.
The hatchery is on Lot 126, Plat of Lake Cushman No. 9. The supply and return pipelines cross Lot
128, Plat of Lake Cushman No. 9 (public boat launch) and continue into unplatted Lake Kokanee. The

B. ENVIRONMENTAL ELEMENTS JhLel
1. Earth
a. General description of the site

Imported gravel surfacing and pipe bedding materials (2,000 cy estimated). The remainder of
the site is a balanced cut and fill where existing topography is within a foot of final grade.

Not likely. The existing site is mostly cleared and the native sandy gravel soils are very free
draining. Regardless, temporary erosion control provisions will be provided.
g. About what percent of the site will be covered with impervious surfaces after project
construction (for example, asphalt or buildings)?

h. Proposed measures to reduce or control erosion, or other impacts to the earth, if any:
A construction storm water pollution prevention plan will be prepared. Generally it will employ
perimeter silt fencing, use of straw bale check dams, a wet pond, and other relevant BMP's.
2. Air
a. What types of emissions to the air would result from the proposal (i.e., dust, automobile,
odors, industrial wood smoke) during construction and when the project is completed? If
any, generally describe and give approximate quantities if known.
During construction equipment exhaust and possibly dust. After constructio i hatchery vehicle
exhaust and hatchery odors. Hatchery odors are usually associated with fis 1 feed and effluent
water treatment.
b. Are there any off-site sources of emissions or odor that may affect your pror osal? If so,
generally describe.
No.
c. Proposed measures to reduce or control emissions or other impacts to air, if any:
This hatchery employs dual-drain circular rearing vessels where fish waste end uneaten food
are swept into a center drain within minutes and go to a clarifier. Also, the fish rearing areas
will be paved with concrete which will be washed down regularly into the clarifier which further
limits the amount of uneaten feed that could produce odors. These actions will direct most of
the odor to the clarifier which will be at the most remote end of the site away from residences,
thus hatchery odor may be unnoticeable off-site.
3. Water
a. Surface Water:
1) Is there any surface water body on or in the immediate vicinity of the site (including year-
round and seasonal streams, saltwater, lakes, ponds, wetlands)? If yes, describe type and
provide names. If appropriate, state what stream or river it flows into.
Lake Kokanee which is an impoundment of the North Fork Skokomish River.
2) Will the project require any work over, in, or adjacent to (within 200 feet) he described
waters? If yes, please describe and attach available plans.
Yes, the water supply and return pipelines lay within Lake Kokanee. ThE hatchery itself is
approximately 300 feet from ordinary high water.

3) Estimate the amount of fill and dredge material that would be placed in or removed from
surface water or wetlands and indicate the area of the site that would be affected. Indicate
the source of fill material.
625 cubic yards of excavation for the pipeline trenches and 550 cubic yards to fill back in
the trenches. Of the 550 cubic yards, 370 cubic yards will be gravel backfill for bedding the
pipelines, and 180 cubic yards will be native glacial outwash that was excavated from the
trench. The original grade of the bottom of the lake will be placed back to original elevation.
4) Will the proposal require surface water withdrawals or diversions? Give general
description, purpose, and approximate quantities if known.
Yes, a 6 cubic foot per second water right has been obtained for use of Lake Kokanee
water. This water will be used for juvenile rearing and adult fish holding.
5) Does the proposal lie within a 100-year floodplain? If so, note location on the site plan.
The hatchery upland site does not lie within the floodplain. The pipelines that traverse Lake
Kokanee are within the floodplain.
6) Does the proposal involve any discharges of waste materials to surface waters? If so,
describe the type of waste and anticipated volume of discharge.
All of the water withdrawn for the hatchery will be returned to Lake Kokanee and
discharged near the inlet for the river discharge at the base of the dam. This is subject to
an NPDES permit currently under review.
b. Ground Water:
1) Will ground water be withdrawn, or will water be discharged to ground water? Give general
description, purpose, and approximate quantities if known.
The hatchery is purchasing ground water from the Lake Cushman Maintenance Company
who obtained a water right to provide Tacoma with 100 gallons per minute. This water will
be used for incubation of eggs. No discharge into the ground except for natural storm
water infiltration and the on-site sewage disposal system.
2) Describe waste material that will be discharged into the ground from septic tanks or other
sources, if any (for example: Domestic sewage; industrial, containing the following
chemicals. . . ; agricultural; etc.). Describe the general size of the system, the number of
such systems, the number of houses to be served (if applicable), or the number of animals
or humans the system(s)are expected to serve.
An on-site sewage disposal system will be constructed for discharging waste from personal
restrooms and utility sinks and will be sized for a maximum 375 gallons per day. The
discharge material will be comparable to residential type discharges.
c. Water runoff(including stormwater):

The storm water system is composed of three elements. All runoff from paved surfaces is
collected in catch basins directed to a storm water filter cartridge treatment system from
which the water is discharged to Lake Kokanee. All runoff from roofs is ollected and
discharged downstream of the filter cartridge system to Lake Kokanee. Runoff from the
gravel boat launch parking expansion is treated in a wet pond and then infiltrates or
overflows to Lake Kokanee.
2) Could waste materials enter ground or surface waters? If so, generally describe.
Yes, however treatment measures are being employed for both storm water runoff and
hatchery effluent as described previously.

The storm water treatment and hatchery effluent treatment systems previously described.

currently forested land that is medium size firs with salal underbrush. A small amount of additional

tank.

None
d. Proposed landscaping, use of native plants, or other measures to preserve or enhance
vegetation on the site, if any:

Native vegetation will be left undisturbed beyond the hatchery and parking lot limits. Grass and
landscaping will be planted between the hatchery and road on the west side.
5. Animals

For the hatchery site there are marbled murrelet. Management of buffer habitat for the
northern spotted owl (Strix occidentalis) is indicated on the PHS list for this area; however,
the proposed action would not result in any effect to this species or its habitat since no
nesting trees would be removed, and forested habitat in the immediate project area is
fragmented and disturbed. For Lake Kokanee there are bull trout; however bull trout critical
habitat is not designated in Lake Kokanee (75FR 63979).
c. Is the site part of a migration route? If so, explain.
Yes, ducks and other migratory birds.
d. Proposed measures to preserve or enhance wildlife, if any:
The purpose of the hatchery is to preserve and enhance fisheries.

Formalin is used as a treatment for fungal diseases and is considered haza dous. Formalin
storage rooms and a formalin dosing room have been specifically designed in accordance with
the building code to house this material.

As discussed above, the rooms that will house formalin are designed in accordance
with building code requirements for this material.

Construction will result in short term equipment noise but hours will be limited to 7 am —6
pm. Operations will result in traffic noise from hatchery vehicles such as for fish hauling but
worker hours will generally be 7:30 am —4:00 pm or a half hour one wa or the other.
3) Proposed measures to reduce or control noise impacts, if any:
None.
8. Land and shoreline use

Current use of the site is boat launch parking. Adjacent to the west is Lake Kokanee boat
launch and community park, north is residential, east and south are forested areas then roads
and residential.

No obstruction created. The hatchery will be in full view from the adjacent road. There will be limited
view of the hatchery from the park since there is a buffer of trees. There will be limited view of the
hatchery from the residence to the north with the buffer of trees filtering visibility.
c. Proposed measures to reduce or control aesthetic impacts, if any:
Siding and roof colors will be chosen that are aesthetically allied with th existing
environment.
11. Light and glare
a. What type of light or glare will the proposal produce? What time of day wou d it mainly occur?
There will be limited exterior lighting at doorways but it is generally not desirable to illuminate
hatchery rearing tanks at night. Exterior switched lighting will be provided bL t will be off after
dusk under normal conditions. In the unusual case of needing to perform some work in the
yard at night, or if there is an intruder, the lights are equipped with dimmers f Dr low level lighting
that can be turned up.
b. Could light or glare from the finished project be a safety hazard or interfere with views?

Cushman No. 2 dam is listed on the national register of historic places.

There are archaeological and culturally significant sites in the Cushman area known only by the Tribe
and persons with a need to know at Tacoma Power. This site has been reviewed by a professional
archaeologist and there are no restrictions on construction except for the standard to stop work if any
discovery is made.

14. Transportation
a. Identify public streets and highways serving the site, and describe proposed access to the

From the Lake Cushman road, take Lower Lake Road (private) to Dow Creek Drive and to the site.
Access to the street will be at the same location as the existing access to th lot on the southwest
corner.

No. Transit stops are available 2 miles away on the Lake Cushman road.
c. How many parking spaces would the completed project have? How many ould the project
eliminate?
The project eliminates random use by vehicles with boat trailers that could accommodate more than 10
vehicles but rarely does. It restores 10 trailer parking spaces in the expand d south end of the site.
Inside the hatchery complex is parking for 15 exterior designated spaces and covered parking for 4
vehicles.
d. Will the proposal require any new roads or streets, or improvements to existing roads or
streets, not including driveways? If so, generally describe (indicate whethe public or private).
No.

served from Tacoma Power in the same underground trench that power is being installed. A
new on-site septic system will be installed on the east side of the hatchery.
