Olympic Region Clean Air Agency(ORCAA)regulations define a demolition project as the wrecking, razing, leveling, dismantling, or burning(by a fire department for training purposes) of a structure, malting the structure permanently uninhabitable or unusable. Renovations include the removing of load bearing structural members, but not to the extent to make the structure uninhabitable. The following information is merely a reference guide and not a substitute for agency regulations. 1. A good faith asbestos survey is required for any demolition. The survey must be conducted by a certified Asbestos Hazardous Emergency Response Act(AHERA)building inspector.Qualified contractors and inspectors may be found in your local Yellow Pages, through the Washington State Department of Labor and Industries,or on ORCAA's website. 2. Asbestos samples must be sent to a NVLAP Laboratory (National Voluntary Laboratory Accreditation Program) per 40 CFR 763.87.A list of labs can be found on ORCAA's website. 3. The start date on other structure demolitions must be at least 10 working days from the submission date of the complete application and payment. 4. It is the responsibility of the property owner and/or demolition contractor to ensure there is no asbestos-containing material present in the structure to be demolished. 5. Any and all structures on the same parcel of property that are not proposed to be demolished must be identified as such. 6. A copy of the asbestos survey and approved Demolition Permit must be kept on site and be available for review by Agency inspection personnel. 7. The original demolition permit will expire 1 year from start date.If the pernbit expires and the project is not complete, you must submit and pay for another demolition permit. Under no circumstances will a project be extended beyond 1 year from original submission date. ADDITIONAL REQUIREMENTS: In addition to Agency requirements, most building departments require a demolition permit(separate from ORCAA's Demolition Permit). The Washington State Department of Labor&Industries and the local fire authorities may also require notification for asbestos removal projects. Project#Lawson.-Demo. Samples were taken for the purpose of determining asbestos containing materials within a single family structure with intent to demolish. The home is a 1981 Double wide manufactured home..The structure is approx. 1440 sq R and has 2 bedrooms and 2 bath. The structure has Wood siding and vinyl windows,Insulation in wall and ceiling of home were fiberglass bat. There is sheet laminate flooring in the kitchen, bathrooms and laundry. There is carpet in the remaining areas of the structure with wood subfloor throughout house. The home has wood paneling walls with the exception of 1 wall in Living room that has sheetrock. The whole house has pressboard ceiling. The roof of the home is composition. Five Guys Excavating conducted an asbestos survey to identify possible ACM's(Asbestos Containing Materials)A total of(3)samples were taken from the structure as shown in the chart below and(0) of the(3)samples were found to contain a regulated level of asbestos. 1.2 SUMMARY Five Guys Excavating conducted an asbestos survey to identify possible ACM's(Asbestos Containing Materials). Sampling was conducted in accordance with sampling and analytical procedures required by Asbestos Hazard Emergency Response Act(AHERA),Olympic Region Clean Air Agency(ORCAA),and the Asbestos Survey Requirements of the State of Washington Department of Labor and Industries(L&I)WAC 296-62-077. 1.2 SAMPLE LOG SEE ATTACHED The owner should also use caution when performing demolition activities within the project areas. Concealed materials may be encountered during the demolition project.ACBMs may be located between walls.above ceilings, underneath floor,in pipe chases,or other inaccessible areas.A copy of the asbestos survey document should be on site during all demolition activities. 2.0 LIMITATIONS OF THE ASSESSMENT As addressed under 2.4 Limitations of the assessment in Project survey#Lawson-Demo The conclusions of the report are professional opinions based solely upon visual site observations and interpretations of analyses as described in our report.Common construction techniques render portions of the building inaccessible. The opinions presented herein apply to the site conditions existing at the time of our investigation, and interpretation of current regulations pertaining to asbestos.Therefore.our opinions and recommendations may not apply to future conditions that may exist at the site. Future regulations pertaining to asbestos material may impose stricter restrictions on demolition work. 2.1 OBJECTIVE The objective of Five Guys Excavating's assessment work was to evaluate the presence of asbestos materials from this residential home prior to a demolition project. 2.2 SCOPE OF WORK The scope of services for the asbestos materials assessment included, and was limited to,the following tasks: • Perform an asbestos inspection to identify asbestos-containing building materials(ACBM) present from this residential home. Sampling was conducted in accordance with AHERA sampling requirements and analyzed by a NVLAP accredited laboratory for the presence of asbestos using Polarized Light Microscopy(PLM). Prepare a summary report documenting the findings of the assessment and providing tables summarizing materials, analytical results, comments and recommendations. 2.3 LIMITING CONDITIONS *Five Guys Excavating's site inspection included a physical inspection of accessible areas_ 2.4 LIMITATIONS OF THE ASSESSMENT •The conclusions of the report are professional opinions based solely upon visual site observations and interpretations of analyses as described in our report.Common construction techniques render portions of the building inaccessible. As a result, additional ACBM may be present in inaccessible areas(e.g.,crawl spaces, above ceiling, and behind walls)at the site. •The opinions presented herein apply to the site conditions existing at the time of our investigation, and interpretation of current regulations pertaining to asbestos. Therefore, our opinions and recommendations may not apply to future conditions that may exist at the site.Future regulations pertaining to asbestos material may impose stricter restrictions on demolition work. 2.5 WARRANTIES *There are NO warranties written, expressed or implied. 3.0 PROJECT SUMMARY Information concerning the subject property was obtained from site inspections conducted by Lisa Demmon of Five Guys Excavating. This section provides a sampling methodology and a brief discussion of Five Guys Excavating's assessment results.Supporting documentation provided within the survey report includes material summary tables and the Appendices section that includes the staff certification and laboratory analysis reports with chain-of-custody documentation. 3.1 ASBESTOS SURVEY METHODOLOGY Five Guys Excavating's asbestos survey was performed following the sampling protocol for the renovation/demolition areas as outlined under AHERA,40 CFR 763,the Olympic Region Clean Air Agency (ORCAA)and the State of Washington Department of Labor and Industries WAC 296-62-077.Sample locations were chosen in a random fashion, with emphasis being placed on obtaining samples of each tvpe of accessible suspect material_Five Guys Enterprise's survey was conducted by Asbestos Hazard Emergency Response Act(AHERA)accredited building inspector,an accreditation through training AHERA,29 CFR Part 763,Lisa Demmon(Certificate Number:011422-04)during the asbestos inspection.Copy of the building inspector certificate is provided in Appendix A. Samples were collected in a non-abrasive manner by carefully removing small portions of the suspect material with a sharp knife or other hand tool suitable to the material being sampled. Each sample was placed in a pre-labeled plastic container immediately after collection.Sample containers were then placed in a large re-sealable plastic bag for transportation to the laboratory. The sampling instrument was subsequently wiped with a clean moist cloth to decontaminate the tool and prevent the potential release of asbestos fibers or contamination of subsequent samples. Data pertinent to each sample(e.g.,date,sample number, material description, and material condition)were recorded on a sample log sheet. The sample log forms are kept on file at the Five Guys Excavating's office. Asbestos bulk samples, copies of the sample log and chain-of-custody submittal sheets were delivered to International Asbestos Testing Laboratories(iATL)in Mt. Laurel,New Jersey. iATL participates in the National Voluntary Laboratory Accreditation Program(NVLAP)for quality control procedures. As specified in 40 CFR Chapter I(1-1-87 edition)Part 763,Subpart F,Appendix A,each sample was analyzed using polarized light microscopy(PLM)/dispersion staining techniques,in accordance with U.S.EPA Method 600/M4-82-020.Detection limits for this type of analysis are approximately one half percent(by volume). Materials containing at least one percent asbestos are considered to be regulated asbestos-containing building materials(ACBM). 4.0 CONCLUSIONS AND RECOMMENDATIONS Five Guys Excavating collected a total of three(3)bulk samples from this residential home and zero(0)of the three(3)samples collected were found to contain a regulated level of asbestos.Asbestos abatement is not required for these items shown in the Certificate of Analysis. The owner should also use caution when performing demolition activities within the project areas. Concealed materials may be encountered during the demolition project.ACBMs may be located between walls, above ceilings, underneath floor,in pipe chases,or other inaccessible areas.A copy of the asbestos survey document should be on site during all demolition activities. Supporting documentation for the asbestos samples, including a copy of the laboratory analysis reports with the chain-of-custody documents are provided in Appendix B. Recommendations for Verudeulite Analysis: Several analytical protocols exist for the analysis of asbestos m vermiculite. These analytical approaches vary depending upon the nature of the vermiculite mineral being tested(c.g.un-processed gauge, homogeneous exfdiated books of mica,or mixed mineral composius). Please contact your client representative for pricing and mmatntatd time options available_ iATL rosin rnerrds initial testing using the EPA 600/R-93/116 method. This method is specifically designed for the analysis of asbestos in bulk building materials.it provides an acceptable starting point for primary screening of vermiculite for possible asbestos Results from this testing may be inconclusive.EPA suggests proceeding to a multi-tiered analysis involving wet separation techniques in conjunction with PLM and TEM gravimetric analysis(F.PA 600:R-04004). For New York State automers.NNSDOII requires disclaimers and qualifiers for vanotts vermiculite containing samples that direct analysis via ELAP198.6 and EL.AP 198.8 for samp"that contain>10°i vermiculite mineral where EIAPi 99_6 may be used to evaluate the asba*K content of the material. However, any test result using El API 98.6 will be reported with the following disclaimer."ELAP198.6 method does not re nave vermiculite and may underestimate the level of asbestos present in a sample containing>10%vermiculite."
