Box 1180 Hoodsport, WA 98548 RE: Little Valley Water System ID# 06161K Dear Mr. Sund, The Mason County Department of Health Services (MCDHS)has received an Application for Determination of Adequacy from John Wyatt. The application indicates that Mr. Wyatt would like to connect to the Little Valley Water System. After review of the Little Valley file,the MCDHS has determined that this system has not yet met the requirements for the additional three connections. The attached letter, dated October 13, 1997, outlines the steps required to obtain full approval for the additional three connections. Mr. Wyatt's lot is not included in the original system layout sketch nor is it indicated in the original notice to future property owners. Once the three items contained in the attached letter are addressed and a nitrate analysis is made,the MCDHS will approve the Little Valley water system for the full six connections. Your nitrate analysis is due every three years and was due in 2003. Box 1180 Hoodsport, WA 98548 RE: Little Valley WS # 06161 K Dear Mr. Sund, This letter is to inform you that the above referenced water system has met all state and local regulations and is hereby approved for 6 connections. The water system will need to have periodic water sampling tests. A bacteriological analysis will be required on an annual basis. The next bacteriological analysis for this water system is due on June 8, 2006. A nitrate analysis will be required every three years. The next nitrate test will be due October 17, 2008. Your Water Facilities Inventory (WFI) form has been sent to the Washington State Department of Health. They will enter the water system into their computer database and assign a unique number to the water system. This number and the full water system name should be put on all correspondence so that identification can be simplified and proper credit given to the water system. Changes in ownership and/or management or the number of connections on the water system must be reported by updating the WFI form for your water system. Copies of the updated WFI must be sent to Mason County Environmental Health and Washington State Department of Health. If you have any questions, please call me at 427-9670 extension 293.
