NW • Seabeck, WA 98380-875I riC DVi�V alliedsepticdesign@gmail.com Page I \ LOT EVALUATION / FEASIBILITY STUDY CLIENT: Accurate Development/Thomas Wolter ■ tom@thomaswolter.com (206) 200-3325 (C) RE: Reassessment of Existing Septic System Installed in 1996 ADDRESS: 391 E Olde Lyme Rd ® Shelton, WA 98584 TAX ID: 32127-53-00217 On September 20, 2022, I evaluated a parcel at 391 E Olde Lyme Rd. in Shelton, Washington, at the request of Thomas Wolter of Accurate Development for a reassessment of an existing septic system installed in June of 1996; SWG93-0645 shows an approved stamp from the Mason County Health Department in 1995. While the original design SWG93-0645 was for a three-bedroom system, only a two-bedroom system was actually installed. I inspected the tanks and observed vandalism to the lids and risers and debris that had been thrown into the tanks. Pump reports attached show rainwater was pumped in 07/21 with trash and debris found on the bottom of tanks. The debris has since been removed and the lids and risers were re- placed. One lateral cleanout line was also damaged at the clean-out port and has also been repaired. Individual line valves were located and a lidded access box installed. EVALUATION Minor repairs have now been made to the damaged components, a valve box was installed over the system line valves and the tanks, which never contained effluent had the groundwater pumped out on 7/22/2021, and tanks were cleared of all debris. A pump needs to be put in the pump tank once the site is secure and electrical controls wired and set up once a home is moved in; even though the riser lids are now screwed down, there is still a concern that scofflaw vandals might reoffend. Have system pumped and remove roots and any foreign objects that are not part of system, IE rocks,sticks.ect. Install a pump and locate ends of laterals and inspect and test drainfieid.
