the Gambriell Bulkhead Repair Beach Nourishment project(SEP2013-00056) located at
23020 North Highway 101 in Ho 2 dsport as proposed by Steven Gambriell. The Department of
Ecology(Ecology)reviewed the vironmental checklist and has the following comment(s):

A floodplain development per it will be required for work in the 100yr floodplain. A habitat
assessment may be required to determine impact to ESA listed species per the National
Marine Fisheries Service/Federal Emergency Management Agency (NMFS/FEMA)
Biological Opinion(BiOp).

CONDITIONS OF THIS DETERMINATION
1) Proposed residential developments adjacent to a water body supporting aquaculture
operatoins shall install drainage and storm water treatment measures facilities to prevent
any adverse impact to aquaculture operations.
2) Residential developers and individuals shall be required to control erosion during
construction. Removal of vegetation should be minimized and any areas disturbed should
be restored to prevent erosion and other environmental impacts.
3) Debris or deleterious material resulting from construction shall be removed from the beach
area and project site and shall not be allowed to enter waters of the State.
4) Water quality is not to be degraded to the detriment of the aquatic environment as a result
of this project.
5) All applicable State and Federal Permits and approvals must be obtained by owner.

ESA LISTED SALMONIDS CHECKLIST
This worksheet was designed to help project proponents, and government agencies, identify when a project needs
further analysis regarding adverse effects on ESA(Endangered Species Act)listed salmonids. Salmonids are salmon,
trout and chars, e.g. bull trout. For our purposes, "ESA Listed Salmonids" is defined as fish species listed as
endangered,threatened or being considered for listing.
If ESA listed species are present or ever were present in the watershed where your project will be located,your project
has the potential for affecting them, and you need to comply with the ESA. The questions in this section will help

The Fish Program Manager at the appropriate Department of Fish and Wildlife (DFW) regional office can

Existing concrete bulkhead has a crack that needs to be repaired before it results in structural
deterioration of the bulkhead. Repair would consist of constructing a concrete column (8' wide x 8'4"
tall x 8" thick) on the waterward side of the bulkhead, tied back to a concrete deadhead and concrete
patio slab behind (landward) of the bulkhead.
After completion of concrete work, beach in area of work (approx. 65 lineal feet along bulkhead) will
be nourished with new round washed rock to elevate the beach grade. Rock will vary in size from 3"
bull rock down to pea gravel. Rock will be brought in from upland facility. Assumes 14 cu. yds. 3"
rock, 14 cu. yds. 1.5" rock, 14 cu. yds. pea gravel.

Concrete forms will be constructed on the waterward face of the bulkhead, up from the existing wall footing.
Minimal (hand) excavation would be required for this work. The area behind the bulkhead will be excavated to
construct the concrete deadhead using a medium sized excavator. Re-bar will be placed in the form, through the
existing bulkhead and back into the deadhead area. Concrete will be pumped into the forms and allowed to
cure, then the forms will be removed and excavated areas filled in with materials previously removed.
The rock brought in for beach nourishment will be placed on the beach with the excavator staged upland of the
bulkhead and spread out on the beach as necessary by hand.
