Updated Date: 12/7/2015 Arrow Septic Designs 171 E.Vuecrest Dr. Union,WA 98592 December 22,2022 Mason County Department of Health Services 415 N 6th St Shelton,WA 98584 RE: Jeffrey Burgess Property(Parcel#32227-53-03007) Septic Evaluation of Failure Dear Inspector: Attached is a replacement/upgrade septic design for a property located at 100 NE Shorebrook Dr,Tahuya,WA 98588. There is a small 1-bedroom cabin that was built inf 950he s and a arne era 3-bedroom house that was built in 1965 on the site. The septic system is a gravity trench system taking At a service visit in December-2022 for the propertyWas found to be saturated, clogged, ponding and hasale,it was discovered tat the drainfield was tnot been water. Upon further investigation, the drainfield operating properly. Records indicate the existig system consists size and location.of The old tank is tolbe removed(and the tank e compartment septic tank followed by a gravity drainfield of unknown hole backfilled with C-33 sand)and the old drainfield is to be abandoned. The proposed 4-bedroom replacement system will include a 500-gallon pre-trash tank followed by a NuWater BNR-500 pretreatment tank and a 1,200-gallon minimum pump tank. The drainfield is to be(2) 10' x 30' shallow pressure beds totaling 600 s.f.. We are also proposing a new control panel with timer, elapse meter and event counter to extend the life of the system and to facilitate future operation and maintenance. This system meets Table IX repair standards with the new proposed drainfield being over 50 feet to the owner's well, treatment level B without disinfection, and 18"(12"minimum)vertical separation. The drainfield is over 25' from the shoreline of Hood Canal. The property owner's contact information is as follows: Jeffrey Burgess 2006 NE 63' St Seattle,WA 98115 (206)450-2640 If you need further information, please contact my office at(360)898-2255.
