PROPOSAL REQUIREMENTS Each proposal should include the following: A. Cover Letter All proposals must include a cover letter to the attention of Kelly Frazier, Facilities Manager, signed by a person legally authorized to bind the applicant to its proposal. Experience with various city, county, police, fire permitting agencies; A list of memberships in professional organizations, special training, licenses and experience shall be included in the submittal. The Contractor must be licensed in WA State. C. Project Team Describe the proposed project team. Who will be the key contacts and leaders of the team and discuss who will interact with the County. Please provide resumes of all key personnel who will conduct any work on this project. Please provide the proposed staffing or total number of persons you plan to have available to assist with regard to this proposal. D. References A professional profile of the Contractor including a list of at least three references from public entities the Contractor has provided similar services to. Include the scope of work, budget, timeline and change order report. IV. EVALUATION PROCESS RFQs are to address, and will be evaluated upon, the following criteria: 1. Experience, Qualifications, & Past Performance Proposal Elements: In a narrative, identify the team (to include working titles, degrees, certificates and licenses), describe the roles and responsibility of each team member, and why each team member is critical to the success of the overall contract. Describe the team's experience and capacity for providing the services being requested in this RFQ. • Provide an organizational chart demonstrating the relationships and hierarchy of the team described above. Identify individuals by name, position, discipline and firm. Identify key back up personnel. • Resumes of the key individuals are to be included as an appendix and are not included in the total page count. Resumes are to be limited to one single-sided, letter-size page. Resumes exceeding this limit will not be reviewed. • Include a list of, at a minimum, three recent contracts/projects in the last five years, to include a point of contact, contact information (phone and email), and brief description, for services relevant to the items listed in the Scope of Services as performed by the key team members. Only projects completed by key members of the project team will be considered. The County will evaluate: • The team's capacity and ability to provide the County with the required services detailed in the Scope of Services. Project Approach Narrative Proposal Elements: Clearly describe the team's general approach and methodology for accomplishing the various services outlined in the Scope of Services section, generally providing civil/structural design and engineering support services. Approach should include information on: • Project Management: Describe how the team will manage and coordinate the necessary disciplines required to accomplish the services requested. • Communication Plan: Project communication for each task between the team, the County's project manager and various stakeholders. • QC/QA: Quality Control and Quality Assurance program for the requested services. Describe the team's experience responding to and overall management of on call/task order services and describe the firm's approach for responding to the County's request for services. • Provide approach for scheduling and sequencing of individual task orders and for providing simultaneous services on multiple task orders and facilitating efficient progress on each task order. • Approach for managing multiple task budgets and approach for ensuring cumulative task budgets do not exceed the overall contract amount. The County will evaluate: The project team's approach for providing professional civil and structural engineering services and for providing efficient management of an on-call contract with multiple task orders. 3. Accuracy and Completeness of the RFQ RFQs will be evaluated on the following criteria: • Formatting and layout. • Spelling and grammatical accuracy. • Legibility of figures and chart information. • Coherent and logical flow of written responses FINAL EVALUATION PHASE (if applicable) 4. References Reference checks will be requested from the project list provided under 1. Experience, Qualifications, & Past Performance. Reference checks may be performed on the apparent selected vendor, if based directly on the RFQs received or on shortlisted firms if interviews are being requested. The County will evaluate the reference checks to assess the proposed team's overall performance and success of previous, similar work. Reference checks will also be utilized to validate information contained in the RFQ. 5. Interviews (as requested by the County) If an award is not made based on the written evaluations alone, interviews will be conducted with the top-ranked proposers. Failure to participate in the interview process will result in the Proposer's disqualification from further consideration. Selection Process Additional information, interviews and or presentations may be required at the option of the County. In no event shall the County or its Board of County Commissioners, or employees be liable for any costs incurred for the preparation of and participation in the submission of responses to this request or subsequent interviews of persons or companies. B. Compliance with Rules Proposers responding to this RFQ must follow its procedures and requirements. Failure to comply with any portion of this RFQ may result with the rejection of your proposal. C. Proposal Withdrawal Any proposal may be withdrawn at any time before the "Proposal Due" date and time by providing a written request for the withdrawal to the Facilities Manager. A duly authorized representative of the submitting entity shall make the request. Withdrawal of a Proposal will not preclude the proposer from filing a new proposal as long as it's prior to the close date. D. Appeals Proposers who wish to appeal a disqualification of their proposal or the award of contract may submit the appeal in writing to the County Administrator, 411 North 5th Street, Shelton, WA 98584 within five (5) working days of the award or disqualification. Disagreement with the process, e.g., scoring by evaluators, or otherwise, is not subject to appeal. E. Ownership of Documents Any material submitted by a proposer shall become the property of the County. Materials submitted after a contract is signed will be subject to the ownership provision of the executed contract. F. Public Record All proposers and information submitted by proposers are not open for public inspection until after the notice of intent to award a contract is issued. Except for exempt materials, all proposers and information submitted by proposers will be available for viewing after the evaluation process is complete and the notice of intent to award is sent to all participating parties. This is a five (5) year permit that was set to expire on December 31, 2017. Mason County submitted a complete permit renewal application package in May of 2017. Although 5 years have passed and a new permit has not been issued, Mason County's Belfair Water Reclamation Facility still operates under the original permit conditions and limits and remains in compliance with Ecology expectations. At this time Ecology is requiring Mason County to reapply for a new permit by June 1st, 2022. The Department of Ecology requires that the application for permit renewal be signed by an executive official. Attached is a draft permit renewal application. It includes information on influent and effluent flows, water quality results, and operations. The County is not proposing any changes in operations or discharges from the existing permit. The monitoring requirements for the new permit are expected to be very similar to the existing permit. BUDGET IMPACTS: The County pays an annual fee to the Department of Ecology for permit oversight and enforcement that is part of the budget. Submittal of the application does not create any additional costs beyond that annual fee from the Department of Ecology. Water Reclamation and Reuse Standards Water reclamation and reuse standards in Washington were developed in accordance with Revised Code of Washington(RCW)90.46.The standards require emergency storage or alternate discharge locations,automated alarms,redundancy of treatment units,and stringent operator training and certification to meet reliability criteria. These standards designate four classes of reclaimed wastewater based on the extent of disinfection and the potential use:A,B,C,and D.Class A reclaimed waters have the most stringent requirements and are described in the standards as follows: ...[water] that,at a minimum,is at all times an oxidized,coagulated,filtered,and disinfected wastewater.The wastewater shall be considered adequately disinfected if the median number of total coliform organisms in the wastewater after disinfection does not exceed 2.2 per 100 milliliters,as determined from the bacteriological results of the last 7 days for which analyses have been completed,and the number of total coliform organisms does not exceed 23 per 100 milliliters in any sample. Surface infiltration of water meeting these water standards is allowed as long as the receiving groundwater continues to meet groundwater recharge criteria established by the State Board of Health pursuant to Chapter 43.20 RCW,and by the Washington Department of Health pursuant to Chapter 70.119A Washington Administrative Code(WAC). Treatment requirements for surface infiltration of reclaimed water include the following: The minimum pre-treatment for ground-water recharge shall be Class A reclaimed water to ensure significant pathogen reduction... The secondary treatment process used to provide oxidized wastewater shall include an additional step to reduce nitrogen prior to final discharge to groundwater. Critical Aquifer Recharge Areas and Wellhead Protection Areas As defined in the Mason County Code 8.52.120,critical aquifer recharge areas are areas that provide an important supply of water to recharge an aquifer used as a source of potable water,and that are vulnerable to contamination.Critical aquifer recharge areas are areas of special concern and are subject to the Mason County health codes.Within Belfair,the Belfair Sub-Area Critical Aquifer Recharge Area has been defined as shown in Figure 3-3. Standards for an environmental permit for a critical aquifer recharge area include: • Implement best management practices(BMPs);implement the Washington State Department of Ecology's Storm Water,Water Quality,Hazardous Waste,Wetland,and Solid Waste Program BMPs and BMPs from the Departments of Health,Agriculture, Transportation,and State Conservation District Office;or ENVIRONMENTAL ASSESSMENT 8.9 BELFAIR WASTEWATER AND WATER RECLAMATION FACILITIES PROJECT • Demonstrate through a best management practices report how other necessary and appropriate mitigating measures will be integrated in the design,installation,and management of the proposed facility or use;and • Provide a written agreement to the County providing that all employees at the site will be notified that the operation lies above an aquifer recharge area and providing annual training regarding all measures set forth by the established BMPs. Within wellhead protection areas(see Figure 3-3),in addition to any other notice requirements,notice shall be provided to the manager of Belfair Water District 1 for any applications associated with an environmental permit for construction in wellhead protection areas of the District's wells. 3.4.2 Existing Groundwater Conditions Groundwater in unconsolidated glacial and alluvial sand and gravel deposits provides most of Mason County's potable water.Precipitation,which averages 64 inches annually,is the primary source of recharge for Mason County's groundwater(Mason County,2007). As described in the Geology section above,the geologic units exposed in the study area include advance outwash and till(Qga and Qgt)in the uplands areas,and liquefaction- prone alluvium and peat deposits(Qa and Qp)in the Union River delta at the head of Hood Canal.These geologic units are not a significant source of drinking water in the study area. Belfair Water District 1 supplies water to residents in the study area from Well 1,Well 2, Well 3,and Well 4 in the study area as shown in Figure 3-3. [Note:Well 3 is located outside of the study area to the southwest.]Only Wells 1 and 2 are within the proposed project footprint.These wells draw groundwater from much deeper aquifers(identified as the Deep Aquifer System in the Belfair Water District 1's Wellhead Protection Plan(Robinson and Noble,2001)located 250 to 400 feet below sea level.Beneath the Qga deposits the stratigraphy consists of about 75 to 150 feet of pre-Fraser confining deposits of low- permeability clay and silt rich aquitard sediments(identified as the Intermediate Confining Materials in the Belfair Wellhead Protection Plan).Beneath these deposits is the Sea Level Aquifer System ranging in depth from 75 to 200 feet below sea level,underlain by low- permeability aquitard sediments of the Deep Confining Materials. Below the deep materials is the Deep Aquifer System which is tapped by Wells 1 and 2.Well 1 and Well 2 are therefore protected from near-surface contamination by two thick,low-permeability aquitards. The wellhead protection zones of the Belfair Water District 1 wells are shown in Figure 3-3. The zones delineate areas where certain businesses,activities,and land uses are restricted. However,because of the significant thickness of the two confining aquitard units above the screened intervals of Wells 2 and 3,contamination from a surface or near-surface source is most unlikely to reach the Deep Aquifer System.
