The 3/4"anchor bolts were tested to a load of 3200 lbs. This was observed by everyone mentitoned above. Upon arrival I met with Alta Rep Dan&Ben with Vector Engineering. Current permit drawings were provided&the structure was walked with the individuals noted above. Building conditions varied from drawing&inspection requirements&Alta intends to put together updated As-Built drawings&submit to vector&the jurisdication for finalapproval. Items that varied from plan included: • Welders did not possess WABO certification nor was a Weld Procedure Specification available,all welds were previuosly welded. • Steel connections did not match the structural drawings&welds were used in lieu of high-strength bolts, shear tabs were excluded,beams coped,etc. • Anchor rods varied in embed depth but were stated to all be at 7"or greater.Epoxy used was common HY-200&a P.O.was given for the product but no special inspection performed. • Weld quality did not meet AWS Table 6.1 visual acceptance criteria.Weld size was deemed a consistent 1/8" Fillet at best but possesed undercut,underun,undersize,excessive convexity/cold-roll,&fit-up potentially in question per above. • Columns were not welded 1/4"fillet, all-around per detail B/S-5 due to fit-up,nor was a seam weld provided. MTC report is provided to cover basic Q.A.&Special Inspection requirements of the building and will provide report,and 'defer to Vector Engineering for review&acceptance. .—... __. __ _ . Parties present for this meeting were Materials Testing and Consulting (MTC) personnel, Dan Florek of Alta Forest Products (Alta FP), and Benton Cook of Vector Engineering (Engineer of Record). The purpose of the meeting was to address required special inspections for the project. MTC provided a field report regarding this meeting that deferred to Vector Engineering Incorporated (VEI) for final acceptance of the project due to differences between planned and as-built conditions. Specifically; the epoxy anchor rods, welding, and final project acceptance (see attached MTC Field Report #F216418). VEI staff returned for another on site meeting/inspection November 18th, 2019. The meeting's purpose was to address the issues noted in MTC's initial report, and to perform an in-situ load test on the post installed epoxy anchor rods. MTC personnel, along with Dan Florek and Brad Prihoda of Alta FP were present for this meeting as well. A post-installed epoxy anchor rod was tested by MTC personnel to a tensile force of 3,200 pounds, which is greater than the maximum factored design load (see attached MTC Field Report #F220032). VEI personnel performed a non-invasive visual inspection of the welds and deemed them acceptable, besides a few minor issues noted on the Welding Procedure Forms completed by VEI personnel during the inspection (see attached WPS Forms). The minor issues noted on the WPS forms were corrected by Alta FP personnel. VEI personnel then updated the original project drawings to reflect the project's as-built conditions (see attached as-built drawings). The field welding of this project was completed by Alta FP employees prior to inspection. In lieu of the standard special inspections for welding, the welds were inspected for general conformance by VEI staff, then reviewed by the engineer of record Benton Cook for final acceptance. Special inspections for this project are in general compliance with the Mason County Special Inspections &Testing Agreement (see attached copy), except the field welding and post installed anchorage as previously noted. The Table of Special Inspections in the approved plan set complies with the Mason County Special Inspections &Testing Agreement as well. Welding was done in general accord with the WPS provided by VEI and approved by the engineer of record. Inspection was visual only. The inspected work appeared adequate after execution.
