This SAA report has been prepared to discuss the findings on your home that was inspected on
May 3, 2019.
The following people were present:
Mr. & Mrs. Joseph Alexander
Mr. Ray Nein, Fleetwood Homes
Aaron Mars, Contractor
Ken Knutson and John McMillan, Dept. of L&I
Bill Curry, American Home Center was at the site but left and was not present during this
investigation.
The date of manufacture for this home was 10/23/2018; the home is a double section home.
HUD Labels are: ORE543769 and ORE543770.
Mason County is the building jurisdiction of record who has approved all installation
requirements and has issued a certificate of occupancy.
The SAA inspection process for consumer complaints looks at four types of failures that need to
comply with the Federal Standards. The four areas are as follows;
• Imminent Safety Hazard; An imminent and unreasonable risk of death or severe personal
injury. Note: It may or may not represent a failure to conform to the Standards.

• Serious Defect: A failure to comply with the Standards. It renders the home or any part
thereof unfit for ordinary use for which it was intended. It results in an unreasonable risk
of injury or death to the occupants of the affected home.
• Defect: A failure to conform to the Standards. It renders the home or any part thereof
unfit for ordinary use for which it was intended.
• Noncompliance: A failure to conform to the Standards. It is not a defect, serious defect or
imminent safety defect.
During this investigation there are no imminent safety hazards or serious defects. Most items
identified are installation issues.
History: The Alexander's purchased their home on 10/17/2018 from American Home Center in
Tacoma, WA. The home was installed at the end of November 2018.
Loose squeaky floors through the home; SCA
Heat duct register in kitchen floor is not stabilized;
Whole house shakes when dog runs through home or someone walks throughout; GCS
Kitchen countertop issues;
Tile backsplash throughout the house is not grouted or sealed;
Bath room sink does not hold water; 4LS

Sliding glass door in bedroom is unable to unlock from the exterior;,
Floor insulation wet under home at master bath at p-trap;,
Bottom board not sealed in several areas where it was cut; Sc,S

Air is coming up from the electrical service panel in the utility room. a.;-)
Several of these items identified on the (MHORFA) Manufactured Homeowner Request for
Assistance form, are cosmetic issues that fall under warranty work. The State Administrative
Agency has documented several cosmetic items but under the SAA we do not monitor cosmetic
items that were caused either by the manufacturer during production of the home or the dealer
during installation. They are identified in this report as observations only. A number of the above
mentioned items were being addressed at the time this inspection was made.
The following items are issues identified by the SAA during this investigation:
(Item 1) There are no manufactured home installer tags affixed to the home identifying the
contractor(s) responsible for this home's installation.
Installation components that require a certified installer to identify their respective work are:
Concrete pads, runners, structural connections, foundation support system, anchoring system,
mechanical/plumbing, skirting/vents/access. No one of these items have been identified by an
installer tag.

Washington Administrative Code 296-150I-0160
Installer certification tag — Placement — Removal.
(1) The installer certification tag must be placed on the home upon completion of the
installation and prior to inspection by the local enforcement agency.
(2) The installer certification tags must be placed on the end of a home section directly above
or below the HUD certification tag or temporarily located in plain sight within three feet of the
home's front entry.
(3) The local enforcement agency may not issue final approval of a home installation until
one or more installer certification tags have been affixed to the home indicating all installation
work was performed by a certified manufactured home installer.

All energy star homes require metal elbows for the crossover duct where it attaches to the
heat duct under the home. The metal seams of the elbow is required to be sealed with a
mastic UL-181B and special attachment is required. There were not metal elbows installed.

5.6.1 Crossover Duct Materials: Crossover duct materials shall:
1. Be labeled R-8 as a minimum insulation value;
2. have an exterior vapor retarder rated at 1.0 perm or less;

3. have an inner liner material which includes an interior spring -steel wire helix bonded within
two layers of 57-gauge mylar polyester film, or approved equal;
4. have all tears, holes and penetrations sealed;
5. include the crossover duct manufacturer's splice kit, if required;
6. have a minimum interior diameter of 10 inches, or as approved;
7. include a sheet -metal elbow of at least 26-gauge to connect the collar to the
crossover duct;
8. include two-inch wide foil tape (NASHUA #322 or equivalent) or a duct sealer for outdoor
applications (HARDCAST HDC RTA-50 or equivalent) for all metal -to -
metal connections;
9. include stainless -steel, worm -drive clamps or nylon straps; and
10. include a belly -bottom tape approved by the manufacturer.
5.6.2 Sheet -Metal or Elbow: The sheet -metal elbow shall be:
1. 26-gauge and secured to the vertical collar with sheet metal screws;
2. sealed at the connection to the vertical collar with foil tape or duct sealer;
3. sealed at all seams of the adjustable elbow, if applicable, with foil tape or duct
sealer; and
4. insulated to a minimum level of R-8.
5.6.3 Crossover -Duct Connections: Connections of the inner liner of the crossover duct
shall be:
1. cut to minimize excess duct length and/or stress at the connections;
2. secured to the elbow with sheet metal screws or other mechanical fasteners; and
3. taped to the elbow or spliced with clamps or straps.
5.6.4 Crossover -Duct Vapor Retarder Connections: Connections of the exterior vapor retarder
of the crossover duct shall be:
1.secured to the elbow, collar or splice with clamps or straps; and
taped to the belly bottom where the crossover duct enters the belly with approved
belly -bottom tape.
5.6.5 Crossover -Duct Support: The crossover duct shall be:
1. supported above the ground by strapping or blocking where clearances permit;
2. installed to avoid standing water;
3. installed to prevent compression and sharp bends which restrict airflow; and
4. installed to minimize stress at the connections.
There is no sheet metal elbow's per Energy Star requirements. Which would include the
complete connection for Neem Energy Star requirements.

(Item 3) Installation
WAC 296-150I-0310 (d) a manufactured home must have a skirting around its entire perimeter.
It must be installed per the manufacturer's installation instructions or if the manufacturer is not
specific, to the standards in this section. Do not enclose with skirting areas under recessed
entries, porches or decks, (whether constructed as part of the home or added on -site) unless
skirting is of the fully vented type and installed so as to allow water to freely flow out from
under the home. Porch areas open to the crawl space area of the home must have ground cover
removed; ground directly below the porch must slope away from the home. It must be vented
and allow access to the under floor area per the manufacturer's installation instructions or per the
standards below if the manufacturer's instructions are not available.
Fleetwood Installation Manual Page 99, Skirting and decks. Homes with open slatted decking
(recessed entries, porches, or decks) must have skirting or foundation behind these areas to
prevent water from draining back under the home.

(Item 4) Master bath plumbing access was not re -insulated and p-trap cover not re-
installed.

(Item 5) Incorrectly placed wedges and shims, there are a number of blocks with loose
wedges and shims.

NOTE: Fleetwood homes had technicians during the inspection and tighten them up. This
is the installer's and dealer's responsibility to make sure all blocks are tight.

(Item 6) There are some perimeter blocks that have been removed that need to be re-
installed these areas were at the front and end areas of the home. It is undetermined if the
floor deflections are being caused by loose supports under the home. 2x6 floor system will also
have deflection as well. A recommendation, installing additional supports in high traffic areas.

(Item 7) STEP 1. REPAIR AND SEAL BOTTOM BOARD: Tears and openings in the
bottom board can result from transportation or installation activities. Inspect for holes and gaps
the entire bottom board, especially areas around service penetrations, crossover connections, pipe
and duct hangers, foundation elements, and the perimeter of the floor. Using approved materials
appropriate for the type of repair, repair the bottom board wherever torn or loosened as follows:
1. Insulate. Replace any missing insulation prior to closure and repair of the bottom board,
paying particular attention to insulation gaps that may have been created at P-traps. 2. Repair
large openings. Repair large openings with a durable patch made of bottom board fabric or other
compatible material and fastened with vinyl bottom board tape held in place by fasteners
installed with a divergent stapler. Seal the edges around patches with foam or mastic. For large
openings, install a rigid backer board behind the bottom board to provide a fastening substrate
for the patch.. 3. Repair small openings. Repair small gaps and tears with a combination of vinyl
bottom board tape, patches, mastic, or foam sealant.

NOTE: SUPPORTING A HOME FOR STORAGE To prevent damage to homes being stored
but not on display (i.e. people shall not be permitted inside the home) for a period exceeding 30
days, locate piers below each I-beam no further than two feet from each end of the home and at
the approximate center of the home length. This home was manufactured 10/23/2018 and
installed 11/27/2018.
WAC 296-150I-0330 What are the requirements for temporary placement of manufactured
(mobile) homes?
Manufactured (mobile) homes placed on temporary display or in storage by a manufacturer,
dealer or distributor in excess of thirty days shall be:
(1) Supported under each main frame beam by supports located within two feet of each end
and within four feet of the front and rear axle and other supports so that no span shall exceed
sixteen feet; and
(2) Made weather tight at any marriage line joint at the roof and wall lines.

(Item 8) Hot Water Pressure Relief Line does not extend to exterior of the home.
WAC 296-1501-0310(h) Hot water tank pressure relief lines must exhaust to the exterior side of
the exterior wall or skirting and must exhaust downward. The end of the pipe must be at least six
inches but not more than two feet above the ground,

NOTE: Water heater drain for the pan, extends out past the perimeter. Water heater
pressure relief line should be in this area.

(Item 9) The long turn 3" LTTY has been installed with no clean out plug being installed.
The back of the 3 inch ABS plumbing LTTY has had black tape covering the 3" inch opening on
the back side. This is a sanitary issue.

NOTE: Cosmetic item:
Kitchen floor register shows deflection in this traffic area, blocking this area would strengthen
deflection when standing near it.

Conclusion:
At today's inspection, most issues identified are a result of improper installation procedures.
These include system's test that are required to be completed, blocking, structural connections. It
is also noted that this home sat on a storage lot in Grand Mound, WA. for approximately thirty
days during the winter without any temporary blocking as required by Fleetwood and the State of
Washington.
American Home Center is responsible for making the needed installation corrections.
1. WAC 296-150I-0160 Installer certification tag by their installers. This included the
concrete contractor.
2. Bringing the home into compliance with the Neem Energy Star requirements.
3. Fleetwood Installation Manual Page 99, Skirting and decks. Homes with open slatted
decking (recessed entries, porches, or decks) must have skirting or foundation behind
these areas to prevent water from draining back under the home.
4. Master bath plumbing access was not re -insulated and p-trap cover not re -installed. (This
may have been done by Fleetwood's technician at the time of this inspection)
5. Incorrectly placed wedges and shims, there are a number of blocks with loose wedges
and shims. (This may have been done by Fleetwood's technician at the time of this
inspection)

Officials) for inspection guidelines for manufactured home inspections along with random pages
of Fleetwood's installation manual.

3. Weigh down. To keep the retarder in place, weigh it down with stones, concrete
blocks, or other heavy, durable materials.
4. Repair tears. Repair any voids or tears in the retarder by patching with like ma-
terial, maintaining a minimum 12-inch overlap and sealing joints with mastic.
STEP 3. INSTALL SKIRTING
Skirting is any structural or non-structural perimeter crawlspace enclosure. Complete
site -built structures (see STEP 5. COMPLETE SITE -BUILT STRUCTURES, p.101)
that abut the home (such as porches, attached garages, and steps) prior to installing
skirting.

Skirting must be of weather -resistant materials or provided with protection
against weather deterioration at least equivalent to that provided by a coat-
ing of zinc on steel of not less than 0.30 oz per sq ft of surface coated.
Skirting made from wood or wood products and used within six inches of
the ground needs to be made of materials naturally resistant to decay and
termite infestation or pressure -treated.
Ventilation openings must be covered for their full height and width with a
perforated (1/4 inch maximum opening in any dimension) corrosion and
weather -resistant covering that is designed to prevent the entry of rodents.
In areas subject to freezing, the ventilation openings must also be of the
adjustable type, permitting them to be in the open or closed position, de-
pending on the climatic conditions.
To design and install skirting, comply with the skirting manufacturer's instructions (if
provided) and the following:
1. Configure skirting. Run the skirting along the perimeter of the home's heat-
ed, conditioned space. Do not enclose with skirting areas under recessed en-
tries, porches, or decks (whether constructed as part of the home or added on
site) unless skirting is of the fully vented type and installed so as to allow wa-
ter to freely flow out from under the home.
2. Fasten skirting. Attach skirting to the home in a manner that prevents water
from being trapped between the siding or trim and the skirting or forced up in-
to the wall cavities trim to which it is attached. Allow for frost heave when in-
stalling skirting in areas subject to frost.
3. Provide ventilation. Unless the skirting has integral ventilation openings that
meet the following ventilation requirements, install equally sized ventilation
openings on at least two opposite sides of the foundation. Size ventilation ar-
ea to equal at least one square foot for each 150 square feet of under -floor
area (or for each 1,500 square feet if a ground moisture retarder is installed
according to STEP 2. INSTALL GROUND MOISTURE RETARDER, p. 98).
Place vents as high above the ground as practical.
4. Install access. Provide an access opening not less than 18 inches wide and
24 inches high and located so that any utility connections located under the
home are accessible.
5. Extend vents, drains, and inlets. Run appliance exhaust vents, combustion
air inlets, and air conditioner condensation drains through the skirting to the
outside and terminate each as instructed in the sections of this manual corre-
sponding to each appliance.

cold surfaces.

I am adding one additional item that I overlooked on my SAA report that needs to be addressed
by American Home Center's installer contractor.
None of the 8" x 8"x 16" concrete blocks located at the marriage line are installed to
Fleetwood's Installation Manual. All marriage line concrete blocks have been installed parallel
to the marriage line.
Fleetwood Installation Manual page 26, requires that marriage line single stacked concrete
blocks less than 54" inches in height, must have long side of concrete block perpendicular to the
marriage line.

Is the dryer vented to the exterior and 1) does it comply with the dryer manufacturer's
specifications; or 2) is metal with smooth interior surfaces used?
Has the testing of water lines, waste lines, and gas lines been verified with the installer?
Are heat pump and air conditioning condensation lines extended to the exterior of the home?
Are the water heater pressure relief valve and pan relief line terminated to the exterior and
then down?
Is water piping protected against freezing per the home manufacturer's instructions?
Or is a heat tape listed for use with manufactured homes installed per the tape
manufacturer's instructions?
