Special inspection reports shall be submitted to the Mason County Building Department, 615 W Alder St, Shelton WA 98584 and available for inspection. Inspection reports shall be completed and submitted to the dept. in a timely manner and shall be submitted prior to the framing and final occupancy inspections. Envirotech Engineering has called out for4,000 PSI concrete. This requires special inspection. Krazan and Associates has been identified to proga ins ect, sample and take test cylinders. X j 14) Any changes in proposed construction shall be reviewed by the engineer or architect of record and submitted in writing to the Mason County Building Department prior to construction. I met with Arlan to discuss the scope of work for this visit and reviewed the onsite and approved plans and details by Envirotech Engineering dated 3/9/18. The scope of work as discussed is to perform special inspection for the slab-on-grade reinforcement,and observe the concrete placement for the garage slab. I walked the slab-on-grade and noted the ASTM A615, Grade 60 reinforcement is in-place at 12"on-center each way and with 1-'/2"clearance. I noted the vapor barrier is in-place with the seam overlapped and tapped off. I did note the reinforcement at the thickened edges to be in contact with the grade material. The contractor raised the reinforcement and cleared the material off the footing at both thickened edge garage door locations. I observed the placement of 63 yards CalPortland concrete Mix 3400 via boom pump and the contractor consolidate the concrete by hand rodding and tools. I obtained a concrete sample for field tests and cast one set of five 4"x8"cylinders. Please see Concrete Report 10618140CRO8142018AC for the field test results and break dates. Purpose and Scope The purpose of our services,as requested by Aaron,was to perform observation of reinforcing steel,and continuous observation and testing of concrete placement at exterior pole footings.Upon arrival,I met with Aaron,who provided me with plans by Mason County Public Utility District Department of Engineering dated January 5,2018.I noted the reinforcing steel and concrete details. Findings After reviewing the plans,I observed as-built conditions at each of the seven exterior pole footings.I observed ASTM 615 Grade 60 steel to be in-place,with eight#6 vertical bars,#4 hoops at 9"on-center,and two#4 hoops at the top. Cast-in-place anchors were also in-place in general conformance with the provided plans. I then observed the placement of 14 yards of Miles concrete Mix#01500 to the exterior pole footings.Concrete was placed by truck chute,mechanically consolidated,and finished with hand tools.I sampled the concrete at the commencement of pour and tested for temperature and slump,which were within tolerances.I then sampled the concrete at the southwest footing from the first truck.I tested the sample for temperature and slump,and cast four concrete compressive strength test cylinders,which were left onsite for initial set. In the scullery,the approved plans included a Type II hood and exhaust system. The system was not installed, and the dishwasher had subsequently (without review and approval)been revised to a unit shown as"vent-less". While the dishwasher listing may not require a specific exhaust system for the dishwasher,the IMC requires that the room, as part of a commercial kitchen, with significant vapor producing sinks and equipment associated with a commercial kitchen, requires exhaust. Prior to the (non-approved) revision, the proposed Type II hood provided the necessary exhaust for the room. Without the Type II hood and exhaust,the room is left without the required exhaust system. The room is also provided with supply-air, which in accordance with the IMC, requires that that the exhaust volume be increased to also exhaust the additional volume of supply air, as air from a commercial kitchen cannot be re-circulated. (See IMC 403.2.1 (particularly#3), IMC 403.3.2.3, IMC Table 403.3.1.1, and IMC Table 403.3.1.1, footnote b.)An exhaust system for the room is required. 13. Handrails, and rail extensions are required at the gym stairs. 14. Mechanical Room 401, all through penetrations of the 1-hour fire- resistive wall assembly(and top of wall joints) require fire-caulk or some other approved form of penetration/joint protection. 15.
