13) In addition to the inspections required in IBC, Section 110, the owner, the engineer or architect of record acting as the owner's agent shall
employ one or more special inspectors who shall provide inspections during construction on the types of work listed under Chapter 17 and as
specified by the design professional. The special inspectors duties &responsibilities shall be as specified in Chapter 17.
Special inspection reports shall be submitted to the Mason County Building Department, 615 W Alder St, Shelton WA 98584 and available for
inspection. Inspection reports shall be completed and submitted to the dept. in a timely manner and shall be submitted prior to the framing
and final occupancy inspections.
Envirotech Engineering has called out for4,000 PSI concrete. This requires special inspection. Krazan and Associates has been identified to
proga ins ect, sample and take test cylinders.

14) Any changes in proposed construction shall be reviewed by the engineer or architect of record and submitted in writing to the Mason County Building
Department prior to construction. All engineering and/or architectural documents are a part of the approved set of plans and shall remain attached
thereto. If docu ents are removed, approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be

The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance

26) This property has been identified in a FEMA (AE) Flood Hazard Area with a Base Flood Elevation (BFE) of 15 ft. In accordance with the FEMA
• and the International Residential Code the finished floor must be at or above 1 foot above the BFE. Prior to footing/foundation wall inspection
the surveyor or design professional of record must provide a written record of inspection. This verification prior to pour ensures the proper
wall height has been met. This document must be present for inspection by Mason County Building Department.

As requested by Arlan,a representative of Hood Canal Builders,I am onsite at 16821 NE North Shore RD Tahuya
WA. I met with Arlan to discuss the scope of work for this visit and reviewed the onsite and approved plans and details
by Envirotech Engineering dated 3/9/18.
The scope of work as discussed is to perform special inspection for the slab-on-grade reinforcement,and observe
the concrete placement for the garage slab.
I walked the slab-on-grade and noted the ASTM A615, Grade 60 reinforcement is in-place at 12"on-center each
way and with 1-'/2"clearance. I noted the vapor barrier is in-place with the seam overlapped and tapped off. I did note the
reinforcement at the thickened edges to be in contact with the grade material. The contractor raised the reinforcement and
cleared the material off the footing at both thickened edge garage door locations.
I observed the placement of 63 yards CalPortland concrete Mix 3400 via boom pump and the contractor
consolidate the concrete by hand rodding and tools. I obtained a concrete sample for field tests and cast one set of five
4"x8"cylinders. Please see Concrete Report 10618140CRO8142018AC for the field test results and break dates.

Purpose and Scone
The purpose of our services was to perform a concrete cylinder pick up. One set of five concrete
compressive strength test specimens were cast on by Aaron Clyde.
Findines
The five concrete cylinders were in-place at their initial set location and did not appear to have been
tampered with or damaged.
Conclusion
I transported to cylinders to our Poulsbo Lab for standard curing and future compressive strength
testing.

Purpose and Scope
I was onsite for sampling of concrete. I met with the subcontractor for the mix design and building permit.
Findings
Areas of pour: Pier footings at perimeter of building.
Concrete placed by chute and consolidated by vibration. One concrete sample was taken and tested for slump and
temperature,and one set of five 4"x8"test cylinders were cast for compressive strength testing.I observed the placment
of 13 yards of concrete Mix#3400 supplied by Cal Portland.
Conclusion
Concrete placement per ACI and WABO Directives.

The purpose of our services,as requested by Don,was to perform observation and testing of concrete placement of 10
columns and a fire place pad.Upon arriving onsite,I met with Don and reviewed the project plans.
Findings
I observed the placement of approximately 13 yards of CalPortland concrete by boom pump,mechanically consolidated,
and finished with hand tools.The concrete was placed in 10 columns and one fire place pad.I took a sample of the
concrete from the northwest column of the house.I tested the concrete sample for temperature and slump,and cast one
set of five 4"x8"concrete compressive strength test specimens.Please see Concrete Report 10618140CRO7312018EB
for mix details,test results,and break dates.
Conclusion
Based on our observations and field test results,the concrete placement is in general conformance with the project plans
and specifications.

As requested by Aaron representing Rawhide Excavating,I arrived onsite at the Skookum Substation project located at
13 W Kamilche Lane, Shelton,WA.This report presents our purpose and scope of services,our findings,and
conclusions.
Purpose and Scope
The purpose of our services,as requested by Aaron,was to perform observation of reinforcing steel,and continuous
observation and testing of concrete placement at exterior pole footings.Upon arrival,I met with Aaron,who provided me
with plans by Mason County Public Utility District Department of Engineering dated January 5,2018.I noted the
reinforcing steel and concrete details.
Findings
After reviewing the plans,I observed as-built conditions at each of the seven exterior pole footings.I observed ASTM
615 Grade 60 steel to be in-place,with eight#6 vertical bars,#4 hoops at 9"on-center,and two#4 hoops at the top.
Cast-in-place anchors were also in-place in general conformance with the provided plans.
I then observed the placement of 14 yards of Miles concrete Mix#01500 to the exterior pole footings.Concrete was
placed by truck chute,mechanically consolidated,and finished with hand tools.I sampled the concrete at the
commencement of pour and tested for temperature and slump,which were within tolerances.I then sampled the
concrete at the southwest footing from the first truck.I tested the sample for temperature and slump,and cast four
concrete compressive strength test cylinders,which were left onsite for initial set.
Conclusion
Based on our observations,the reinforcing steel and concrete placement at the referenced locations was performed in
general conformance with the project plans provided during this visit and the project specifications. �,

As dispatched I am onsite at the Skookum Sub Station, 121 W Kamilche Lane, Shelton,WA to pick up
concrete cylinders cast by David W.on 8/9/18.I located the samples in the same location as placed for initial curing,
along the south fence line. The samples do not appear to have been tampered with or damaged. I obtained the four
4"x8"cylinders and transported them to our Poulsbo lab for continued curing and future compressive strength testing.

12. In the scullery,the approved plans included a Type II hood and
exhaust system. The system was not installed, and the dishwasher had
subsequently (without review and approval)been revised to a unit
shown as"vent-less". While the dishwasher listing may not require a
specific exhaust system for the dishwasher,the IMC requires that the
room, as part of a commercial kitchen, with significant vapor
producing sinks and equipment associated with a commercial kitchen,
requires exhaust. Prior to the (non-approved) revision, the proposed
Type II hood provided the necessary exhaust for the room. Without the
Type II hood and exhaust,the room is left without the required exhaust
system. The room is also provided with supply-air, which in
accordance with the IMC, requires that that the exhaust volume be
increased to also exhaust the additional volume of supply air, as air
from a commercial kitchen cannot be re-circulated. (See IMC 403.2.1
(particularly#3), IMC 403.3.2.3, IMC Table 403.3.1.1, and IMC Table
403.3.1.1, footnote b.)An exhaust system for the room is required.
13. Handrails, and rail extensions are required at the gym stairs.
14. Mechanical Room 401, all through penetrations of the 1-hour fire-
resistive wall assembly(and top of wall joints) require fire-caulk or
some other approved form of penetration/joint protection.
15. Main mechanical/boiler room requires that all penetrations of the

22. At the restrooms at the commons,not all lavatories are functioning
correctly.
23. A sprinkler head is not correctly mounted/secured at Room 103A.
24. Room 111 classroom door requires adjustment to allow for full
opening width(and proper closing).
25. The two exits at the east side of the building require (moved or
additional) exit signs such that they are visible from within the hallway
that they serve.
26. Room 200C, mechanical equipment above the doorway is
incomplete.
27. Restroom HIOA is not provided with pipe wrap (or boots) at under-
sink knee space.
28. Provide copies of the HVAC balancing and commissioning reports.
29. Exterior grounds/grading remains incomplete. (obviously weather
dependent)
30. Emergency egress lighting test has not been observed or verified to
illuminate upon primary power loss. Has this been tested? If so,please
provide a copy of the report.
