BEFORE A PERMIT CAN BE ISSUED: The registered Design Professional in responsible charge shall fill out the Agreement
and Include the name and address of the company or inspector. Two (2) copies of this form are to be submitted to the
County prior to the issuance of a building permit. If changes are made as to who will perform the special Inspections a new
form shall be submitted and turned Into the Building Permits&Inspections Division for approval. i

STATEMENT 'OF SPECIAL INSPECTIONS: In addition to this Agreement, a "Statement of Special Inspections" shall be
provided per IBC 1704.3. This Statement shall be made as part of the approved plans, and be placed in a conspicuous
location,such as the first page of the construction plans or the first page of the structural sheets.
SPECIAL INSPECTORS: All special inspectors shall be approved by the Building Division prior to performing any duties. The
special inspector shall provide proof of certification as a special inspector for each inspection Item and be licensed by the
State of Washington.
SPECIAL INSPECTION REPORTS: Special inspection reports are to meet the requirements of ISC 1704.2.4. Copies of each
report are to be sent to the address listed in the letterhead noting the Project Address and Permit Number. A final report
shall be submitted stating that all special inspection and structural testing Items were completed and are in conformance
with the approved design drawings and specifications. Items not in conformance,unresolved Items,or any discrepancies in
inspection coverage(i.e. missing Inspections, periodic inspections when continuous was required,etc.) shall be specifically
itemized in the final report.

Special Inspections and Material Testing Proposal
Dear Cory:
Materials Testing & Consulting, Inc. (MTC) thanks you for the opportunity, and respectfully submits the following proposal to
provide materials testing and special inspection services during construction of the above-referenced project. All services will be
provided on a time and materials basis. Although dependent on the actual construction schedule and Sequencing,D1TC will only bill
for actual work-performed,regardless of the raxouuts estimated. Our project manager will review weekly budget status reports,and
invoices will be sent on a monthly basis. No work will be performed beyond the scope and cost estimate without your prior
authorization.
Each of MTC's laboratories are regularly audited by the American Association for Laboratory Accreditation(A21-A)as well as the
Washington Association of Building Officials(WABO). We are specifically recognized to meet the requirements of ASTM C1077,
D3666,D3740,E329,and AASHTO R-18 standards for agencies engaged in construction materials testing. Our proposed inspection
team includes WABO certified senior special inspectors with decades of collective experience and together have worked on thousands
of projects throughout the Northwest totaling over$4 Billion in construction costs.
In order to provide cost reductions and more efficiency to your project, and to reduce our global footprint, MTC provides the
advantage of electronic reporting, distribution, and storage of all the inspection reports produced for your project(s) through our
secure, online system called the Digital Paper Route(DPR). The DPR is an online report distribution and project management tool
which not only enables full-time,web-based access to all of our testing and inspection reports,but also provides daily updates of the
project budget allowing our clients to review both overall contract amounts and "drill down"within a project to further evaluate
specific inspection or lab tasks. The DPR service is provided free of charge to all our clients and their appointed project team.
Additional information and further illustration of this service is included in this proposal.

Observed the placement of approximately 19 CY of Cal Portland concrete mix design#3400,28 day strength @ 5000 psi.Required
strength @ 4500 psi.
Concrete was placed into the foundation forms by truck chute,consolidated by high frequency mechanical vibrator and hand finished with
trowels.
MTC performed temperature and slump testing in accordance with project specifications.One set of 5-4"x8"cylinders were cast for
compressive strength testing.Please reference sample report#C48195 for additional information.
To the best of my knowledge,work performed that was visually inspected was in conformance with the approved plans dated 05-10-2017.
