Box 536 Belfair, WA 98528 Re: Variance request to Chapter 173-160 Washington Administrative Code (WAC) r construction of a water well at 501 NE Haven Lake Drive in the SW `A NE `/4, Section 30, Township 23 North, Range 2 West, W.M, Mason County, Tahuya, Washington ax Parcel # 223305000102) Dear Mr. Amende and Mr. Davis: This letter is in response to your variance request dated September 9, 2010 from Davis tilling, concerning the construction of a water well for at the address, listed above. You are req esting a variance to the Minimum Standards for the Construction and Maintenance of Wells, Chapter 173-160 WAC. Specifically, your request is to construct a water well about 67 feet and 70 feet from drainfields on adjacent properties. This variance request is necessary due to the location of wells and the limited land area available to be developed. After an investigation, interview, and review of information available, a variance is her y granted to Chapter 173-160-171 (3) (b) (iv) WAC. This variance is granted under the ffillowing conditions: I. Well construction must start within 90 days of receipt of this variance. The construction must be completed within 120 days of receipt of this variance. 2. You must comply with Mason County rules and regulations. 3. All well construction shall be performed by a licensed driller as set forth by Chapter 173-162 WAC. 4. A notice of intent to construct a water well must be filed and the fee paid in acc rdance with Chapter 173-160-151 WAC. 5. The well shall be a minimum of 100 feet deep. No other openings in the casing, except a Washington State Department of Health approved pitless-style adapter, installed as recommended by the manufacturer, shall be shallower than 100 feet below land surface. This 100 foot minimum depth condition may be subject to revision based on local geologic conditions. 6. Due to the close proximity of a proposed drainfields, the surface seal shall extend continuously from 30 feet deep to land surface. The seal shall provide casing stability and have a minimum diameter of four inches larger than the nominal size of the permanent casing. During the installation of the surface seal the driller must use a temporary casing, at least 4-inches larger in diameter than the permanent well casing diameter. Temporary casing should extend the entire length of the surface seal prior to installation of any sealing product. Open-hole drilling of the surface seal and sealing without a temporary surface casing IS NOT APPROVED. The driller must contact Mason County and Ecology 24 hours prior to installing the surface seal. 7. Bentonite for the surface seal must be installed as required in Chapter 173-160-231 (2) and(3) WAC. The recent changes to Section -231 require the following: "The annular space must be kept at least one-half full of sealing material whenever the production casing is advanced." 8. The driller must submit a Water Well Report describing the construction to Ecology (Southwest Regional office) within 30 days after completion of the work. Attach a copy of this variance to the well report. 9. The driller must inform Ecology (John Pearch, 360-407-0297) two business days before +. any well construction or decommissioning work begins. The driller must make the site accessible to Mr. Pearch to inspect any and/or all well construction or decommissioning work. 10. The driller must inform Mason County (Arlene Hyatt, 360-427-9670 ext 293) two business days before any well construction or decommissioning work begins. The driller must make the site accessible to Ms. Hyatt to inspect any and/or all well construction or decommissioning work. 11. With the exception of the provisions set forth (above) in this variance, all federal, state, and local requirements shall apply.
