ENVIRONMENTAL CHECKLIST
Purpose of Checklist:
The State Environmental Policy Act(SEPA),Chapter 43.21 RCW,requires all governmental agencies to
consider the environmental impacts of a proposal before making decisions. An environmental Impact statement
(EIS) must be prepared for all proposals with probable significant adverse impacts on the quality of the
environment. The purpose of this checklist is to provide information to help you and the agency identify impacts
from your proposal(and to reduce or avoid impacts from the proposal,if it can be done)and to help the agency
decide whether an EIS is required.
Instructions for Applicants:
This environmental checklist asks you to describe some basic information about your proposal.
7 Governmental agencies use this checklist to determine whether the environmental impacts of your proposal are

significant,requiring the preparation of an EIS. Answer the questions briefly,with the most precise information
known,or give the best description you can.
You must answer each question accurately and carefully,to the best of your knowledge. In most cases,you
should be able to answer the question from your own observations or project plans without the need to hire
-b experts. If you really do not know the answer,or if a question does not apply to your proposal,write "do not
know"or"does not apply". Complete answers to the questions now may avoid unnecessary delays later.
Q Some questions ask about governmental regulations,such as zoning,shoreline,and landmark designations.
Answer these questions if you can. If you have problems,the governmental agencies can assist you.
The checklist questions apply to all parts of your proposal,even if you plan to do them over a period of time
or on different parcels of land. Attach any additional information that will help describe the your proposal or its
environmental effects. The agency to which you submit this checklist may ask you to explain your answers or to
provide additional information reasonably related to'determining if there may be significant adverse impact.

Dear Mr. Robbins,
The County received your DNR SM-6 form and SEPA checklist in December 2003,
however to date we have not received the other information and fees needed to process
these documents. Please also refer to my letter dated September 15, 2003. As we cannot
process these forms as submitted I am returning them to you until you are ready to submit
all of the necessary information, applications and fees. I forwarded the Mason
Environmental Permit Application in September. If you need another copy they are
available from the County Permit Assistance Center(427-9670 ext. 281 or 284).
Enclosed are the new fees for 2004. As previously discussed some of the items needed
would be a Mason Environmental Permit Application, Geotechnical Report, accurate site
plans and copy of the project restoration plan. The new planner for your area is Tammi
Clark, and she may be involved in future review of this proposal,however I will also be
available for questions and information.

Dear Mr. Robbins,
We recently met on site to look at your proposed gravel mine expansion in Eldon WA.
Due to steep slopes on and near the site a Geotechnical Report will be needed to evaluate
slope stability, seismic and erosion issues. The Mason County Resource Ordinance
Landslide Hazard Areas section, 17.01.100 (enclosed)requires a Geotechnical Report in
accordance with subsection E. 5. for land modification, grading, clearing etc. in landslide
hazard areas with slopes greater than 40%. It appears from my brief site visit that the
proposal would meet these criteria and trigger the need for a Mason Environmental
Permit. Mason County would also be the lead agency for SEPA review.
I am enclosing the applications materials that I believe are necessary for the project. I
will contact you if there is any other information that will be needed. A SEPA
determination will be issued when all of the applications materials are completed and
reviewed.

Dear Mr. Robbins:
Attached are forms for your expansion of the Department of Natural Resources (DNR)
Surface Mine Reclamation Permit mentioned above located in a portion of Section 27,
Township 24 North, Range 3 West, W.M., Mason County, Washington.
Please begin as soon as possible with the County or Municipality Approval for Surface
Mining (SM-6). Once you have completed this form, take it to Mason County planner
Scott Longanecker. He will advise you on your next steps for SEPA review.
Then begin working on your maps and cross-sections (see SM-8A form for all items that
must be on the maps). When you have a handle on the size and depth of your site, you
will be able to better answer the questions on the SM-8A. A narrative will be needed to
discuss additional information that cannot fit on the SM-8A. The narrative should also
include a soil budget (calculations of the amount and how you will use the sub-soil and
topsoil to grow trees on the site post mining). If you have any questions, I will be happy

At your request Agua Tierra Land and Water Services, Inc(Agua Tierra)completed an assessment of potential landslide
hazards associated with the addition of two shed roof structures at each end of an approved/permitted pole barn.
The entire structure and the additions lie within 300 feet of a steep slope and is therefore subject to Mason County
landslide hazard area review requirements. A prior geotechnical report was prepared, submitted, and approved by the
County for the pole barn. The shed roof additions were completed subsequent to the original geotechnical report.
This current letter provides an evaluation of the shed roof additions for inclusion as an addendum to the original
geotechnical report. A summary of our findings is presented below. Our Engineer's site visit notes are attached below.
Also,we discussed this Addendum approach for the presentation of our findings with Mason County permitting staff. We
were advised that this Addendum would provide an efficient means for review of this relatively minor modification.
Our scope of work included a site inspection, a review of prior geotechnical report data, discussions with the client and
County staff, and preparation of this letter/report.
In summary, following our evaluation, we found that the subject additions to the pole barn do not adversely impact the
slope. Further,we provide the following statement per County ordinance(17.01.100, Section E.7):
...the hazards of the landslide area can be mitigated in such a manner as to prevent harm to property and public
health and safety. Further,the subject shed roof additions to the pole barn as constructed/observed on the day of
inspection are not expected to have any significant environmental impact.

A shop/pole building was construction along side of an existing farm storage structure,the originally
design was a 42 by 32 enclosed structure with two bays. During construction it was decided to add a 14
foot lean-to on each end for equipment storage.
Visitation on the afternoon of September 15,2008:
1. The building site has been used for equipment storage for over 100 years. The natural terrain had a
low bench along the toe of the hill. Some time in the past some of the bench was removed to make a
level site. Behind the southwest corner the cut is a couple of feet while behind the southeast corner it
is not noticeable. However,the toe of the hill is not near the cut but 50 feet from the southwest corner
of the building and the lean-to. The toe is 25 feet from both the building's southeast corner and the
east lean-to's corner. Therefore the conditions,for the main structure are the same as the lean-to
additions.
2. It was suggested that the drain for the rain gutter on the back of the building be route to the front of
the building for discharge,where the ground slopes away from the structure at a greater rate. Also the
roof gutters for the lean-tos should be constructed to discharge away from the structure on the north
side.
