MacAuthur Tacoma, WA 98465 Dear Mr. Barrett: Re: Approval of Your Variance Request for a Well Constructed Less Than One-Hundred Feet from a Known Source of Contamination, 10731 Northshore Road, Belfair, NWh, SWh, T22N, R3W, Mason County. I have reviewed your variance request to Section 173-160-205 Washington Administrative Code (WAC) to locate a well approximately 75 feet from a septic drain field and 50 feet from a septic tank. You are requesting this variance because your current water supply is a spring which tests positive for coliform, and you would like to replace this source with a drilled well. The property fronts on Hood Canal and is approximately 60 feet by 80 feet. The proposed well site is across the road from the waterfront property, and is upslope from the drainfield. Based on the information provided in your variance request, a variance is hereby granted to construct this well. This approval is granted subject to the following provisions: 1. This approval applies only to use of this well as a single domestic well, and does not waive any other state or local requirements (WAC 173- 160-095) . I would encourage you to contact Mason County Department of Health Services prior to construction of this well, especially if you will be applying for any septic or building permits, or if you plan on using this well for two or more hook-ups. 2. The annular space to be grouted shall be a minimum four (4) inches larger than the casing being installed. The well annular seal shall extend to at least thirty feet below ground surface, or shall comply with sealing requirements as specified in Chapter 173-160, Washington Administrative Code (WAC), whichever is greater. The well shall be sealed with high-solids bentonite or cement grout. The seal shall be emplaced by using a pump, a tremmie tube, or equivalent method that prevents bridging. 3. The well shall be located at least 70 feet from the nearest septic drain field. 4. This well shall be constructed by a drilling contractor licensed in the State of Washington. 5. A well construction notification form must be submitted together with the applicable fee at least 72 hours prior to commencement of well construction. I am enclosing a form for your convenience. . Except as provided by this variance approval, the well shall be constructed in accordance with the Minimum Standards for Construction and Maintenance of Wells, Chapter 173-160 Washington Administrative Code (WAC) . Chapter 173-160-125 WAC provides that all final written decisions of the Department of Ecology made pursuant to this chapter may be appealed to the Pollution Control Hearings Board in accordance with Chapter 43-21B RCW. Procedures for appealing this variance may be obtained by writing the Department of Ecology at the following address: Department of Ecology Water Resources Program P.O.
