Community officials who are aµdmorized by local law or ordinance to provide floodplain management information may also complete this form. For Zones AO and A(without BFE's),a building official,a property owner,or an owner's representative may also provide the information on this certification. SECTION A Property Informmation The Elevation Certificate identifies the building,its owner and its location. Provide the building owner's-name(s),the building's complete street addres&and tit and block number If the property address is a Waal route or PO box number,provide a legal description or an abbreviated location description based on distance from a reference pant. . SECTION 8 Flood Msurance Rate Map Indormatbon In order to properly complete the Elevation Certificate,it is necessary to locate the building on the appropriate FIRM,and record the appropriate information. To obtain a FIRM,contact the community or call 1-800-333-1363. The Elevation Certificate may be completed based on either the FIRM in effect at the time of the certification or the FIRM in effect when construction of the building was started. items 1-6.Using the FIRM Index and the appropriate FIRM panel for the community,record the community number.panel(or page) number,suffix,and index date. From the appropriate FIRM panel,locate the property and record the zone and the BFE(or flood depth number)at the building site. BFE's are shown on a FIRM for Zones Al-A36 AE,AH,Vt 430,and VE.flood depth numbers are shown for Zone AO. Item 7.Record the vertical datum system to which the elevations on the applic�le FIRM are referenced. The datum is specified in the upper right comer of the tide block of the FIRM. (2) Have structural components capable of resisting hydrostatic and hydrodynamic loads and effects of buoyancy; (3) Be certified by a registered professional engineer or architect that the design and. methods of 'construction are in accordance with accepted standards of practice for meeting provisions of this subsection based on their development and/or review of the structural design, specifications and plans. Such certifications shall be provided to the official as set forth in Section 4.3- 3 (2) . (4) Nonresidential structures that are elevated, not floodproofed, must meet the same standards for space below the lowest floor as described in- 5.2-1(2) . (5) Applicants floodproofing nonresidential buildings shall be notified that flood insurance premiums will be based on rates that are one foot below the floodproofed level (e.g. a building f loodproof ed to one foot above the base flood level will be rated as at the base flood level) . Planning Department Review The planning Department will evaluate the proposal and write up an evaluation as to what sections of the ordinance that the proposed development must comply with. Also an elevation certificate will be supplied to the applicant if not already done no. 4. Building Review An elevation certificate must be started. In order to complete this certificate a surveyor must visit the site twice. The first visit will be to establish the required base flood elevation and mark the required height. The second trip will be just before the foundation is poured to verify that the first floor will be located at or above .the required elevation. Before any further inspections are completed the elevation certificate must be completed and stamped by the surveyor and returned to a building official. Once the building has been certified by the surveyor to be at or above the required elevation then construction and inspections can continue. In areas where there is no determined Base Flood Elevation. 1. Early review if possible 2. Building permit application received 3. Planning review Same as above. 4. Building review In these areas there will be no surveyor required. However, the first floor must be elevated 2 to 3 feet above grade and the building inspector will be required to certify this on an elevation certificate. Also, buildings must comply with the design standards in the applicable sections of the ordinance.
