The permit to construct a pile supported dock shall be approved with the following conditions: * The maximum overall length of the dock shall be only so long as to obtain a depth of minus 5 feet, as measured from ordinary low water. * Pressure treated wood pilings shall be completely dry prior to placement in water. In addition, pilings shall be treated with preservatives other than creosote or pentachlorophenol . * A valid Hydraulic Project Approval must be obtained from the Washington State Department of Wildlife and all conditions of that, permit must be complied with. For more information contact Stephan Kalinowski, Habitat Biologist, at (206) 858- 7217. * The date or dates of construction will be provided to the Mason County Planning Department (c/o Steve Nichols) , at least 1 week prior to construction. These proceedings will entail, but not be limited to, the removal of any unauthorized structures and the complete restoration of the site. SEE IMPORTANT GENERAL PROVISIONS ON REVERSE SIDE THAT ARE ALSO PART OF THIS APPROVAL. 1. The use of equipment within the wetted perimeter shall be held to an absolute minimum. 2 . Drive mechanisms of wheeled or tracked equipment shall not enter the wetted perimeter. 3 . Alteration or disturbance of bank, shoreline, , or bank and shore- line vegetation shall be held to an extreme minimum, and all dis- turbed areas shall be protected from erosion and revegetated. 4. The only permissible removal of aquatic vegetation is that inci- dental to the physical movement of piles into the substrate. 5. Extreme care shall be taken to assure no deleterious material such as fresh cement, concrete, lime, petroleum products, chemicals, etc. , be allowed to enter the water. 6. Material treated with penta, creosote, or other preservatives shall be completely dry prior to water encroachment. 7. There shall be no concrete poured into the water. 8. Debris from this pro ect is to be removed and disposed of by placing beyond high wafer flows. This ap roval authorizes dock construction only. Any additional work within the ordinary high water line will require a separate Hydraulic Project Approval. 10. If styrofoam floats are used, they should be fully covered or wrapped to prevent damage by otters and muskrats. 11. There is to be absolutel no removal of logs or large woody debris from the lake bo4om without the approval of Department of Wildlife's habitat biologist on a case-by-case basis. NOTE: This permit covers only those projects that are exempt from the SEPA process. Any projects that require SEPA determination from the county will require a separate Hydraulic Project Approval from the Department of Wildlife. NOTE: This approval relates to the protection of aquatic habitat only. The dock structure will be supported by 8 driven piles . The structure will extend 48 feet from a concrete bulkhead on the property.
