Enclosed is the completed DARPA,required application materials, and site plans. Pursuant to my conversation with Chuck McCoy on October 16, 2007, Mason County will invoice WDFW for the applicable permit fees. The Haven Lake boat launch has been operated and managed by WDFW to provide water access and recreational fishing opportunities for the public since 1956. The proposed work is for normal repair and maintenance of an existing facility with no expansion beyond the original footprint. The proposed project includes the following activities: • Replace the existing outdated toilet with a modern vault toilet that includes access provisions for compliance with the Americans with Disabilities Act(ADA). • Grade and the gravel parking area and entranceway. ADA accessible provisions include paving one parking space. At this site we would like to install a CXT brand vault toilet. These are simple structures without running water, lighting, or any automated fixtures. The waste is secured in a concrete vault tank. We routinely pump the vaults on a designated schedule based on usage rates. Expected use will include recreational visitors and anglers during daylight hours only and no camping is allowed at the site. Mr. Jim Scholz February 4, 2008 Page 2 Attached are drawings and further specifications for the CXT single vault toilet, which include the following details: • The CXT has a minimum vault containment system to hold at least 1,000 gallons of waste for 15,000 uses per vault. • The vault's primary containment system is that of concrete and the secondary system is composed of securely fastened ABS/750 plastic sheeting, which meets National Sanitation Foundation requirements. Additionally, there is a 10-year warrantee for material and structural failure. • The vault is watertight. • The system meets the Washington Department of Health, Water Conserving On-site Wastewater Treatment Systems:Recommended Standards and Guidance for Performance, Application, Design, and Operation and Maintenance.
