It is understood that the On-Site Sewage Disposal System is
contingent upon careful construction in conformance with plans and
specifications submitted by Thornton Land Surveying, Inc. to Mason
County Environmental Health Department.
1 . I, the undersigned, my assigns and successors are aware the
On-Site Sewage Disposal System drainfield has a limited life without
reasonable care and maintenance.
2. That, Thornton Land Surveying, Inc. recommended using the
Ecological Developers Incorporated Grease Extractor and Grease Extractor

3. I agree not to hold the Mason County Environmental Health
Department nor Thornton Land Surveying, Inc. liable in any event that
the On-Site Sewage Disposal System fails to operate and agree to make
any necessary repairs in conformance with the latest regulation of
Mason County Environmental Health Department.

Sether Construction
The evaluation of Public Fire Proeection takes into account many
factors, one of which is density.
We feel that further development in the Lakeland Village area and
the commercial strip in Allyn without the benefit of an approved
hydrant system will severely impact on the ability of the fire
protection district to provide adequate suppression services.
This condition may adversely affect the cost of fire insurance
in the area and the fire district's present insurance rating.
This office recommends the following:
1. Reference: Fire Dist. #5 letter dated July 23,
1980, that a fire hydrant be provided " requested,
its location by approval of the Fire Chief.
2. That the developer in this project participate in
developing proposals to mitigate the area's fire
control and suppression impacts.

Project: Construction of a restaurant/office building (7,000
square feet - 2 stories).
Location: Allyn, Washington, on State Highway 3.
The following comments are made on the above referenced
commercial project, see Section II of checklist.
} (1) Earth (b)(c) Grading, paving and overcovering of the soil
would result in change of ground surface relief.
(3) Water (b) The proposal would result in changes in absorp-
tion rates, drainage patterns, or the rate and amount of surface
water runoff, due to increased impervious surface area resulting
from parking pavement and the actual building itself. Surface
} water and storm water runoff should be contained on-site.
(4) Flora (c) The proposal would result in introduction of new
species of flora into the area, or in a barrier to the normal
replenishment of existing species.
Landscaping and vegetative buffers to diminish usual impacts
r , coming from the office/restaurant complex and its associated
parking areas is recommended. Landscaping of this centrally
located proposal will enhance the project and add to the pleasant
Allyn community experience.
(7) Light and Glare The proposal will result in light and glare
from the building because the restaurant will 'most likely be open
in the evenings. Increased traffic generated because of the
project will increase light and glare on SR-3 which will affect
surrounding residential uses along the traffic corridor. Parking
lot and exterior building lighting will also add to impacts.

(8) Land Use The existing vacant grocery in Allyn in front of
Lakeland Village Shopping Center is the site of the proposed
restaurant/office complex. Linear mixed use development along
SR-3 characterizes Allyn. The immediate site is already commit-
ted to commercial development with the shopping complex behind
the site providing a focus when driving through the community. A
vacant lot and residential dwelling are directly south of the

Introduction: The State Environmental Policy Act of 1971, Chapter
43, 21C, RCW, requires all state and local governmental agencies to
consider environmental values both f'or' their owit'actions and when
licensing private proposals. The Act also requires that an EIS be
prepared for all major actions significantly affecting the quality of
the environment. The purpose of this checklist is to help the agen-
cies involved determine whether or not a proposal is such a major ;
action.
Please answer the following questions as completefy' as you' can,with
the information presently available to you. Where explanations of
your answers are required, or where you believe an. explanaCiori would
be helpful to government decision makers, include your explanation in
the space provided, or use additional pages -if necessary . You should
include references to any reports or studies of which you are aware

III. SIGNATURE
I , the undersigned, state that to the best of my knowledge

site. To the north is Lakeland Drive, a primary collector acces-
sing residences in Lakeland Village Subdivision. A tire store
and fire district #5 are sited to the north. The Jolly Tavern is
directly accross the highway from the site.
An addition of a restaurant/office will serve to attract
more tourists and commuters to Allyn for meals while enjoying the
charm of Allyn. An office building, assuming it shall be for
professional uses, could be a community asset which would attract
a stronger economic base in the area and enable the Allyn commun-
ity to have the option of staying closer to home for professional
services.
(11) Population The Allyn community has a suitable population
base to support the proposal. The Belfair - Allyn area has
experienced growth increases of about 8% annually since 1974.

The addition of this office/restaurant will generate more
people in Allyn while they use the services being provided.
Depending on hours and type of restaurant and offices the
population will vary.
(13) Transportation/Circulation The Mason County Parking
Ordinance requires 1 parking space/200 square feet for
restaurants. Which equals 18 spaces for this 3,500 square foot
restaurant. Professional offices require one space/150 square
feet which equals 23 spaces for this 3,500 square foot office
space. A minimum of 2 handicap spaces must be provided. A total
of 53 spaces are necessary.
The proposed site plan identifies 43 parking spaces. 6 of
the 43 spaces are shown in a parking area which is used by the
Lakeland Village Shopping Center, also.
The existing site plan identifies 2 access points onto the
site. The first is off SR-3. The second accesses to the
existing private driveway to the Shopping Center.
A more reasonable alternative to the SR-3 access would be to
provide access off of Lakeland Drive thereby decreasing potential
circulation bottlenecks and traffic hazards on SR-3. Re-design
of the parking area would be necessary to enable this re-design
to occur.
The parking ordinance also requires on-site retention of
surface water, paving and sight obscuring vegetative screening
from residential uses as well as landscaping. Planting native
shrub and tree species would enhance the visual appeal of the
development and invite passerbys to stop and visit the complex.
Especially important would be the type and design of landscaping
adjacent to SR-3. It would be necessary to limit the height of
buffering so as not to decrease traffic visability in the area,
yet soften the visual parking impacts associated with the
proposal.
(14) Public Services The proposal will require fire flow and
protection as well as police protection. Fire District #5 is
located in Allyn and would answer any fire emergency. The Mason
County Sheriff's Department would be answering to any police
emergency.
(15) Energy PUD #3 out of Belfair would be. providing
electricity for this proposal.
Water would be provided by Allyn Center Shopping Center
System.
A community septic tank drainfield sewage system would be
utilized which would probably necessitate a community maintenance
agreement.
