The Lead Agency for this proposal has determined that it does not have a
probable significant adverse impact on the environment. An Environmental
Impact Statement (EIS) is not required under RCW 43 .21C. 030 (2) (c) . This
decision was made after review of a completed Environmental Checklist and
other information on file with the Lead Agency. This information is
available to the public upon request. MITIGATION MEASURES ARE
ATTACHED.
This DNS is issued under 197-11-350 (2) , the Lead Agency will not act on
this proposal for 15 days from the date below. Comments must be
submitted by 07/05/95.

CONDITIONS FOR
SEP95-0135
BULLDOG ASSOCIATES - GRADING PROJECT
1) The applicant shall establish temporary sediment and erosion
control measures on site prior to commencing with the
grading approved through a Mason County Land Modification
Permit. These measures shall be effective until an approved
stormwater water quality plan is implemented. Construction
erosion and sediment control elements, detailed in the Irwin
Engineering grading plans, shall be implemented.
2) An approved permanent stormwater water quality plan shall be
prepared in accordance with the DOE Stormwater Manual and
approved by the Mason County Dept. of Public Works. This
plan shall include the detention pond for infiltrating and
treating surface runoff.
3) The applicant shall apply to the Dept. of Ecology for
coverage under the State Baseline General Permit for Storm
Water Discharges Associated with Industrial Activities.
Many of the elements of this Permit will be set forth in the
permanent stormwater water quality plan stated above.

the Kitsap Bank fulfills the bonding requirement for this permit.
This signed permit will register your access, with the State as a Type "D" non commercial
approach.
Please keep this permit with your permanent files.
Upon completion of work on this approach you will need to make a written request for
inspection and release of the escrow funds as provided in Special Provision No. 22 to the Area
Maintenance Superintendent. It is your responsibility to provide any release of escrow form

Public Works has reviewed and commented on the US plan and Permanent
Stormwater Plan prepared for Len Perry's Lakewood Plat "I" and found them to
be acceptable (letter dated June 14, 1994) . The basis of Public Works review
and underlying requirements are the Puget Sound Water Quality Authority
Technical Manual . With the Technical Manual as the foundation for Public Works
review, the Lakewood Plat "I" should be in compliance with EPA NPDES
requirements for stormwater discharge.

Randy,
I've reviewed the proposed stormdrain facilities proposed
for Lakewood Plat "I" , prepared by Harry Irwin and feel the
proposal is acceptable under Ecology and Mason County standards.
The cover letter for the engineering is dated May 24,
1994, and the revised plans show revision dates of May 23, 1994.
An operation and maintenance agreement should be prepared
for the completed facility. Inspections should be performed at
least twice a year. Attached please find an example O&M covenant
that I am considering as a model for future developments and
facilities. Recording of this covenant with the plat (or on the
face of the Plat Map) should satisfy our need to ensure continued
functioning of the proposed facility.
Elements of a Temporary Erosion & Sediment Control Plan
have been incorporated into the Plan submitted but the adequacy
of the measures offered is difficult to determine at this time.
Additional silt fencing may be required at locations where
clearing and grubbing occurs within 50 feet or less of adjoining
neighboring properties or near the top of steep slopes. The three
cul-de-sac locations may require erosion control features. The
General Notes for Erosion & Sediment Control on Sheet 1 of 8 of
the Drainage Plan and Silt & Erosion Control Plan calls for (item
4) a pre-construction conference between interested parties. I
feel this conference should be conducted on site, at which time
the project engineer can justify the placement or lack of
utilization of E&S measures.

Thank you for the opportunity to comment on the determination of
nonsignificance to complete roads within a plat and provide
underground utilities, proposed by Bulldog Assocfates '(95-0135) .
We reviewed the environmental checklist and have :the following
concerns.
Any work in or adjacent to waterways that will adversely affect
water quality must receive specific prior authorisation from the
Department of Ecology pursuant to WAC 173-201A-110. A short-term
water quality standards modification may be issued if the
proponent agrees to a number of specific construction practices
and techniques designed to minimize water quality impacts. The
construction schedule will be tied to the schedule on the
Hydraulic Project Approval (HPA) .

Access will be staked and flagged by Permittee for pre-inspection. Culvert will lay in line of ditch.
Access will be pre-inspected by this office prior to installation. Culvert to be minimum of 12"diameter by 30'length.
