The old creosote bridge was destroyed by the December 2007 storm and has been removed from the site. PROVISIONS 1. TIMING LIMITATIONS: The project may begin July 15, 2008 and shall be completed by September 15, 2008. 2. NOTIFICATION REQUIREMENT: The Area Habitat Biologist(AHB) listed below shall receive written notification (FAX or mail)from the person to whom this Hydraulic Project Approval (HPA)is issued (permittee)or the agent/contractor no less than three working days prior to the start of construction activities. The notification shall include the permittee's name, project location, starting date for work, and the control number for this HPA. 3. Work shall be accomplished per plans and specifications submitted to the Washington Department of Fish and Wildlife entitled 'Twanoh State Park- Proposed Replacement Bridge" except as modified by this Hydraulic Project Approval. A copy of these plans shall be available on site during construction. BRIDGE 4. The bridge structure shall be placed in a manner to minimize damage to the streambed and banks. 5. The bridge shall be constructed to pass the 100-year peak flow with consideration of debris likely to be encountered. 6. Abutments, piers, piling, sills, approach fills, etc., shall not constrict the flow and cause any appreciable increase (not to exceed 0.2 feet) in backwater elevation (calculated at the 100-year flood) or channel-wide scour, and shall be aligned to cause the least effect on the hydraulics of the stream. 7. Fresh concrete or concrete by-products shall not be allowed to enter the stream at any time during this project. Where aggregate or earth type material is used for paving or accumulates on the bridge, curbs, or wheel guards shall be installed and maintained to prevent the loss of this material into the stream. 9. Approach material shall be structurally stable and shall be composed of material that if eroded into the water shall not be detrimental to fish life. EQUIPMENT 10. Equipment used for this project shall not enter or operate below the ordinary high water line. 11. Equipment used for this project shall be free of external petroleum-based products while working around the stream. Accumulation of soils or debris shall be removed from the drive mechanisms (wheels, tires, tracks, etc.)and undercarriage of equipment prior to its working below the ordinary high waterline. Equipment shall be checked daily for leaks and any necessary repairs shall be completed prior to commencing work activities along the stream. WATER QUALITY 12. Erosion control methods shall be used to prevent silt-laden water from entering the stream. These may include, but are not limited to, straw bales, filter fabric, temporary sediment ponds, check dams of pea gravel-filled burlap bags or other material, and/or immediate mulching of exposed areas. 13. Alteration or disturbance of the bank and bank vegetation shall be limited to that necessary to construct the project. Within seven calendar days of project completion, all disturbed areas shall be protected from erosion using vegetation or other means. Within one year of project completion, the banks shall be revegetated with native or other approved woody species. 14. All waste material such as construction debris, silt, excess dirt or overburden resulting from this project shall be deposited above the limits of flood water in an upland disposal site. 15. If high flow conditions that may cause siltation are encountered during this project,work shall stop until the flow subsides. 16. If at any time, as a result of project activities, fish are observed in distress, a fish kill occurs, or water quality problems develop (including equipment leaks or spills), immediately stop work except for efforts to control leaks or spills or prevent toxic substances from entering the water and notify the Washington Military Departments Emergency Management Division at 1-800-258-5990, and to the Area Habitat Biologist listed below. Work shall not restart on the project until approved by the Area Habitat Biologist. Additional authorization from other public agencies may be necessary for this project. The person(s)to whom this Hydraulic Project Approval is issued is responsible for applying for and obtaining any additional authorization from other public agencies(local,state and/or federal)that may be necessary for this project. All Hydraulic Project Approvals issued pursuant to RCW 77.55.021 (EXCEPT agricultural irrigation,stock watering or bank stabilization projects)or 77.55.141 are subject to additional restrictions,conditions or revocation if the Department of Fish and Wildlife determines that new biological or physical information indicates the need for such action. The person(s)to whom this Hydraulic Project Approval is issued has the right pursuant to Chapter 34.04 RCW to appeal such decisions. B. Environmental Elements 1. Earth a. Genera/description of the site: Flat, rolling, hilly, steep slopes, mountainous, other: b. What is the steepest slope in the site (approximate percent slope)? The steepest slope in the project area (e.g. construction site) is 5%. ENVIRONMENTAL CHECKLIST - Continued 3 C. What general types of soils are found on the site (for example, clay, sand, gravel, peat, muck)? If you know the classification of agricultural soils, specify them and note any prime farm land. Soils in the sewer system area are sand and gravel. Soils in the drainfield area include sandy topsoils over sand and gravel. d. Are there surface indications or history of unstable soils in the immediate vicinity? If so, describe. No. e. Describe the purpose, type and approximate quantities of any filling or grading proposed. Indicate source of fill. The purpose of the grading associated with the proposed project is to replace unsuitable or displaced volume for the sewer system construction. An estimated 2,700 tons of suitable gravel backfill will be used for the sewer construction. Drainfield construction will require approximately 2,000 CY of excavation and 1,000 CY of sand backfill. However, design criteria will be finalized based on flow data from the new flow meter to be installed at the lift station. f. Could erosion occur as a result of clearing, construction, or use? if so, generally describe. Most of the site areas have been cleared. Some erosion can be expected during construction associated with excavation, but erosion control measures will be implemented to control/minimize it. Silt fences, straw bales, and limits on stockpiling will be incorporated into project documentation. g. About what percent of the site will be covered with impervious surfaces after project construction (for example, asphalt or buildings)? There will be no change in use, and therefore none. xLess than one percent of the project site will be covered with impervious surfaces. The only impervious surface associated with this project is a new 450 square foot lift station. The existing lift station building will be used for storage. h. Proposed measures to reduce or control erosion, or other impacts to the earth, if any: At a minimum, silt fences and hay bales will be used. Disturbed areas will be hydroseeded. What types of emissions to the air would result from the proposal(i.e., dust, automobile, odors, industrial wood smoke) during construction and when the project is completed? The only impacts to air quality associated with this project will be exhaust emissions from gas and diesel powered construction equipment. No long-term impacts to air quality are anticipated. b. Proposed measures to reduce or control emission or other impacts to air, if any: Construction equipment will be operated and maintained to minimize emissions. 3. Water a. Surface 1) Is there any surface water body on or in the immediate vicinity of the site (including year-round and seasonal streams, saltwater, lakes, ponds, wetlands)? if yes, describe type and provide names. If appropriate, state what stream or river it flows into. Twanoh Creek flows through the Park area to Hood Canal. Twanoh Creek is freshwater and Hood Canal is a large saltwater fjord off Puget Sound. 2) Will the project require any work over, in, or adjacent to (within 200 feet) the described waters? If yes, please describe and attach available plans. The majority of the sewer system replacement will occur along existing alignments more than 200 feet from Hood Canal or Twanoh Creek. Replacement of the existing lift station will require work within 200 feet of Twanoh Creek. 3) Estimate the amount of fill and dredge material that would be placed in or removed from surface water or wetlands and indicate the area-of-the that would be affected. Indicate the source of fill material. No filling or dredging in wetlands or surface waters is proposed. 4) Will the proposal require surface water withdrawals or diversions? Give general description, purpose and approximate quantities if know. No. ENVIRONMENTAL CHECKLIST - Continued 5 5) Does the proposal lie within a 100-year floodplain? if so, note location on the site plan. According to FEMA Flood Insurance Rate Map 5301 1 501 50C (See attached FIRM), the only portion of the Twanoh State Park in the 100-year Flood Plain lies along the shore of Hood Canal. The 100-year floodplain does not extend above the Ordinary High Water Mark of the beach, or up Twanoh Creek. 6) Does the proposal involve any discharges of waste materials to surface waters? if so, describe the type of waste and anticipated volume of discharge. No. b. Ground 1) Will groundwater be withdrawn, or will water be discharged to groundwater? Give general description, purpose and approximate quantities if know. Temporary dewatering of trenches may be required during construction. 2) Describe waste material that will be discharged into the ground from septic tanks or other sources, if any (for example: domestic sewage, industrial, containing the following chemicals . . .; agricultural, etc.). Describe the general size of the system, the number of such systems, the number of houses to be served(if applicable) or the number of animals or humans the system(s) are expected to serve. The Park currently disposes of its wastewater to ground. It collects wastewater from septic tanks and its collection system and pumps to a drainfield about one-half mile to the south. The proposed plan would modify the existing drainfield with sand beds as part of Phase II construction. C. Water Runoff(including storm water) 1) Describe the source of runoff(including storm water) and method of collection and disposal, if any (include quantities, if known). Where will this water flow? Will this water flow into other waters? if so, describe. The existing campground area contains catch basins and curtain drains discharging to Twanoh Creek. This proposal does not result in permanent changes to the stormwater system, once construction is complete. According to the dye study conducted by the Department of Health, it appears that waste material is currently entering surface waters. Once project construction is completed and the new facilities are operational, waste materials should not enter ground or surface water. d. Proposed measures to reduce or control surface, ground and runoff water impacts, if any: One of the primary purposes of the proposed project is to replace the Park's aging collection system with one that is watertight. This will eliminate any existing ground and surface water impacts and prevent water quality impacts generally associated with aging septic system infrastructure. As such, many of the structures in Twanoh State Park are more than 75 years old; most built by the Civilian Conservation Corps. None of these structures will be impacted by the proposed sewer system maintenance project. According to State Parks Archaeologist, State Parks has initiated four archaeological surveys within Twanoh State Park. None of the surveys have uncovered subsurface cultural resources.The project scope is limited to replacement of sewer lines, one lift station and other related underground infrastructure. The proposed project is the replacement of the existing concrete block anchor systems on 275 moorage buoys at State Parks throughout Puget Sound,Hood Canal, Strait of Juan de Fuca, Rosario Strait,Harney Channel, and San Juan Channel, see the attached list of moorage buoy locations and figures 1 to 11. The proposed project will replace the existing concrete block anchoring system with a helical anchor system,see attached figures 12 and 13. The project will consist of the removal of the concrete block anchor systems and the installation of a helical anchor systems. All work will be performed by State Parks staff using hydraulically driven motors secured directly to the auger, or depending on depth of water,with added extensions to drill the anchor into the bottom. PLEASE EXPLAIN ANY SPECIFIC NEEDS THAT HAVE INFLUENCED THE DESIGN, State Parks is proposing to replace the existing moorage buoy anchor systems for the following reasons: • The helical anchor system is the best available, environmentally friendly mooring technology that exists today. • Recent listing of salmon under the Endangered Species Act is causing all agencies to take a closer look at how their facilities may be impacting salmon habitats and the habitats of forage fish salmon feed upon. • The Washington Department of Fish and Wildlife is currently reviewing policy changes for anchor designs they are willing to approve. Fish and Wildlife has instructed us that our design impacts salmon habitat and will not meet policy acceptance once a new policy is approved. Because our buoy systems are serviced every two years,our maintenance, and therefore our buoy design, is subject to regulatory review and required permits by WDFW and the Army Corps of Engineers. Replacement with better technology systems must occur or Parks will not be able to obtain permits for maintenance. 7c. DESCRIBE THE POTENTIAL IMPACTS TO CHARACTERISTIC USES OF THE WATER BODY. THESE USES MAY INCLUDE FISH AND AQUATIC LIFE,WATER QUALITY,WATER SUPPLY, RECREATION,and AESTHETICS. ATTACH A SEPARATE SHEET IF ADDITIONAL SPACE IS NEEDED. The proposed project will have long term benefits to fish and other marine life in the project areas. The existing concrete block anchor systems can scour the surrounding vegetation and sea bottom. The new helical anchor system will allow marine vegetation to reestablish in areas surrounding the moorage buoy anchors and improve fish and other marine wildlife. Some short term turbidity may occur in the immediate area surrounding the buoys during the removal of the existing anchors and the installation of the new anchors. It is expected that each new anchor system can be installed in approximately one to three hours. Disruption to the bottom is minimized with this type of Installation as the unit is not hammered or driven into the bottom. The noise factor for installation would be comparable to a small gas driven generator running to provide power to a light system. A riser buoy, approximately 10" long and 6"wide, is installed on the line 10 feet up from the anchor end of the buoy line to prevent the line from dragging on the ocean bottom. Some temporary disturbance to nearshore feeding could occur to fish species during buoy anchor removal and installation. Removal of the old and plaMment of the new buoy anchors could result in displacement of sediments causing minor sediment suspension. Fish in the area would likely move away fro the site temporarily due to the activity and should experience no effects from temporary sedimentation. Direct effects on wildlife attributable to anchor removal and installation activities,consisting of engine noise, are short-term and of minor magnitude. It could be expected that wildlife, such as foraging eagles, may avoid the project areas during moorage buoy anchor removal and installation. To minimize impacts to fish and wildlife State Parks will comply with the following recommendations: 1. If buoys are located in water with a depth of less than-30 feet(MLLW=0.00)then the buoy shall meet the following design criteria: • The line between the anchor and surface float shall not exceed thew water depth as measured at extreme high tide plus a maximum of 20% additional line. • The buoy system shall include a subsurface float designed to keep the line between the anchor and surface buoy from contacting the bottom during low tidecycles. The subsurface float shall be located off the bottom a distance equal to the line length as defined above. • An embedment style anchor shall be used in order to avoid impact to eelgrass associated with existing surface anchor designs. 2. The buoy system shall be designed and deployed in such a manner to prevent a moored vessel from grounding out at low water. 3. Project activities shall not degrade water quality to the detriment of marine life. 4. Eelgrass and kelp shall not be adversely impacted due to project activities. 5. The anchors shall not be installed in vegetated shallows(eelgrass beds)or designated herring spawning habitat. 7d.
