I am checking with our accounting department to see if the check that was sent with the original application has been cashed. Also, enclosed are copies of the DARPA application, SEPA Exemption determination, BPA, and the Corps of Engineers NWP issued for the project. PROVIDED that: 1. A telemetry system acceptable to Parks and this Department is required for this project. Specifications must be amended to include this provision prior to seeking bids. 2. A representative of this office must be notified to complete a final inspection of this project and witness a pressure test of the distribution system prior to covering the drainfield. 3. A final Operation and Maintenance Manual (with As-Built) must be prepared and stamped' by a licensed engineer and submitted to this office for approval within 60 days following construction completion. 4. Within sixty days following completion of and prior to use of this project, the attached certification(Construction Report) must be completed and stamped by a professional engineer and returned to this department. WAC 246-27213-08001 provides that if the Certification of Completion has not been submitted within two years of the date of this letter, this approval becomes null and void unless you request an extension in writing at that time. In accordance with an interagency agreement between Parks and Department of Health, the owner will not be invoiced for plan review fees or inspections.
