WFI Discrepancies
Dear Commissioner Jensen:
The State of Washington Department of Health, Southwest Drinking Water Operations has
received the recently updated Water Facilities Inventory (WFI) form for your water system,
which was signed by you. Unfortunately, we are unable to process the WFI due to the
following reason:
➢ Decrease in population that will change Group Type will not be processed without a
letter.of explanation for the decrease. Why did the population decrease so
drastically from 75 per day to 10 per day?
Please resubmit the WFI, along with a letter of explanation, to our office for processing. Until
this matter can be cleared up, please be sure to monitor according to the schedule at the
bottom of the WFI to prevent any violations from occurring. I can be reached at (360) 664-
