The drywell had been dug up, and effluent was observed surfacing over the lid. 6-23-95 Water sample taken by Mark Tompkins, effluent surfacing out of drywell 2 ,400 fecal coliform per 100 ml . 7-17-95 Failure turned over to Kim Lincoln for repair. 7-24-95 Initial letter mailed out to property owner. 8-9-95 On site meeting with property manager, designer, and Kim Lincoln. 10-6-95 Repair Permit received. 10-11-95 Site evaluation by Kim Lincoln. 12-11-95 Design received. 12-12-95 Appeals to the issuance of the permit were received by both Janice Cooperstein and Mrs. V.E. Cokelet. 12-12-95 Design stake-out inspection, conducted by Kim Lincoln and Mark Tompkins. 12-14-95 Design approved by Mason County Department Health Services. 12-21-95 Appeal Hearing. 12-27-95 Letter received by MCDHS, from Janice Cooperstein. * Letter states additional concerns. 1-4-96 Health Officer determination sent out. 1-5-96 Letter dated 1/2/96 received by MCDHS, from Steve Wecker, witness for Janice Cooperstein. * Letter detailed his concerns regarding the design. 1. Waste Strength 1-22-96 Letter received by MCDHS, from Janice Cooperstein. * Letter detailed her concerns regarding design flows . 1-23-96 Letter received by MCDHS, from Janice Cooperstein. * Letter detailed more concerns regarding design. 1. Maintenance & Operation 1-30-96 Letter sent to Michael Dick, from MCDHS. * Letter states that new water readings indicate flows were underestimated and the design must be re-submitted. 2-10-96 Letter received by MCDHS, from Bob Slee, property manager for Michael Dick. * Letter, and follow-up phone confirmed, water saving device will be implemented throughout the building. In the phone call it was agreed that after the devices are in place monitoring of the water use will be done by the designer. 3-28-96 Phone call from Bob Slee and designer Al Everson. * A X-Ray machine used by the dentist was found to be using 70 gallons of water an hour. * This machine was updated and modified to be recycling. 3-28-96 Letter received by MCDHS, from Janice Cooperstein. * Letter detailed more concerns regarding design. 1. States repair is an expansion. 4-11-96 Design Submitted. 4-19-96 Design reviewed and failed. 4-29-96 Letter received by Dr. Trucksess, MCDHS Health Officer, from Janice Cooperstein. NOTE: THIS LETTER WAS IMMEDIATELY FORWARDED TO MCDHS DUE TO CONFLICT OF INTEREST SEE LETTER SENT 5/2/96. * Letter details more concerns regarding design. 5-2-96 Letter sent to Janice Cooperstein, from Brad Banner. * Letter states that Dr. Trucksess forwarded her letter of 4-29-96 to him due to a conflict of interest. 5-10-96 Design submitted, and review, placed on hold for additional information. 6-26-96 Design conditionally approved. 6-30-96 Appeal submitted by Janice Cooperstein. 7-1-96 Letter sent to property manager, designer, installer stating conditions of approval . 7-2-96 Phone call to Brad Banner, from Bob Slee property manager. * Mr. Slee was inquiring about what regulation we have prohibiting the installation of a approved design, even if it is under appeal . 7-2-96 Memo faxed to Mr. Slee regarding phone call inquiry. * Memo states MCDHS has no regulations regarding this. But any installation should be done at the owner' s full risk. 7-7-96 Appeal submitted by Vivene Cokelet. 7-8-96 Final inspection, failed. * Approved for cover only, still need inspection of pump tank, and alarm panel . 7-11-96 Requested information sent to both appellants . 7-16-96 Final inspection, pass . 7-25-96 Letter received by MCDHS, from Janice Cooperstein. * Letter states grounds for appeal . 1. Design is deficient. 2 . Repair is an expansion. 3 . System is too large for site. 4 . Owner does not have easement from her so the system can be installed and maintained. 5 . Installation involves risk or harm to and trespass over the adjacent properties. 7-29-96 Installer called MCDHS, Roofing contractor had driven over and damaged portions of the front disposal area. * Maintenance permit applied for and damaged portions replaced. 8-9-96 Letter received by MCDHS, from Steve Wecker R.S . , witness for Janice Cooperstein. * Letter details his concerns with the design and installation. 1. Design flows 2 . Expansion, not repair 3 . Operation & Maintenance inadequate 4 . Installation problems 8-22-96 Appeal Hearing before Health Officer. 8-28-96 Notified by phone that the high water alarm had sounded. 9-5-96 Final Conclusion of Health Officer sent to appellants. . . . I uphold the Mason County Health Department' s approval of the on-site septic system. . . The following conditions do apply: 1. No automatic override mechanism is allowed. 2 . Fill Soil impacting Vi Cokelet' s property must be bulkheaded or mediated in some acceptable manner. 9-19-96 Letter sent to Bob Slee, Property Manager, from Kim Lincoln. * Letter notified owner that MCDHS had not yet received the required information from the O&M professional regarding the alarm sounding. 9-19-96 Received fax from Eric Knopf, O&M professional . * Notification of alarm sounding, observations made and actions taken. 9-19-96 Notified automatic override removed. 9-20-96 Complaint received from Mrs . Cokelet. * complainant state she was smelling an odor. 9-20-96 Complaint investigation by Grant Holdcroft . * Unable to detect any odor or problems . 9-30-96 Entire building was checked for leaks . Several toilets were still having problems. * All seven were replaced with new low flow models. 10-28-96 Bulkhead along Cokelet property completed. 11-1-96 Received copy of letter form Eric Knopf, O&M professional, to Bob Slee, Property Manager. * Letter states O&M professionals observation of the system.
