ATTACHED.
This DNS is issued under 197-11-350 (2) , the Lead Agency will not act on
this proposal for 17 days from the date below. Comments must be
submitted by 09/18/95 .

GRD95-0033 LAND MODIFICATION PERMIT
JACK AND STEVE JOHNSON SEPTEMBER 26, 1995
1) 1'he applicant shall establish temporary sediment and erosion control measures on site
prior to commencing with the grading approved through a Mason County Land
Modification Permit. These measures shall be effective until an approved stormwater
water quality plan is implemented. Construction erosion and pediment control
elements, detaijed in the Olympic Engineering grading plans, shall be implemented.
2) detailed site plan and permanent stormwater water quality plan shall be prepared in
accordance with the DOE Stormwater Manual and approved by the Mason County
Dept. of Public Works. These plans shall include the retention ponds for treating and
i,,ifiltrating surface runoff. The applicant shall sign and notarize a Declaration of
Covenant (Operation and Maintenance Agreement) concerning storm drainage
facilities, and submit it to Mason County Public Works within 30 days of permit
issuance.
3) 1'he applicant shall agree to the following conditions concerning the borrow site(s)
and source of fill materials:
a) the applicant shall notify Mason County Department of Community Development
of the location and amount of expected fill to assure that critical areas (steep slopes,
wetlands, streams, shorelines) are not affected; and
b) source fill materials shall not contain organic debris (tree or vegetation materials)
or solid/hazardous materials (construction or household wastes. petroleum products,
or chemicals);

4) Future development (land uses, structures and paved/parking areas) of this project
area shall require approval by the Mason County Dept. of Public Works for the
adequacy of sto 'water facilities.
5) Future development (land divisions) of this project area shall be subject to further
SEPA public view by Mason County Dept. of Community Development.
