If this changes a full an complete set of plans detailing the despening operation will need to be sub tted for review and permitting prior to the installation of the system. A separate submittal is required for review, per section 4604.3 of the 2006 In ernational fire code. X��f/'Z-? 2) PER TITLE 14 MASON COUNTY BUILDING CODE -CHAPTER 14.17, STANDARDS FOR FIRE APPARATUS ACCESS ROADS - 14.17.110: A fire apparatus access road in excess of 14% grade and more than 150' to new residential or commercial structures will require an automatic fire sprinkler system installed. Contact the Mason County Fire Marshal at(360) 427-9670, extension 273, for further information. y 3) Proper disposal of construction debris must be on land in such a manner that debris cannot enter Hood Canal, Alderbrook Creek or cause water quality degredation of state waters. # 4) Any discharge of sediment-laden runoff or other pollutants to waters of the state is in violation of Chapter 90.48, Water Pollution Control and WAC /� 173-201A, Water Quality Standards for surface waters of the State of Washington and is subject to enforcement action. COM2008-00114 2 of 5 5) The recommendations made within Section 8 of the Biological Assessment/Evaluation prepared by Allison Reak of Sea-Run Consulting and dated December 2007 with Addendum dated April 2008, are to be implemented throughout the project. Water quality is not to be degraded to the detriment of the aquatic environment as a result of this project. ;, -- Erosion control measures must be in place prior to any clearing, grading or construction. These control measures must be effective to prevent stormwater runoff from carrying soil and other pollutants into surface water or storm drains that lead to waters of the state. Sand, silt, clay particles and soil will damage aquatic habitat and are considered to be pollutants. 8 Construction debris must be safely managed, especially if it contains painted wood or concrete, treated wood or other possibly dangerous materials. All dock materials not suitable for re-use will be disposed of at a permitted upland waste facility. 9) During construction, all releases of oils, hydraulic fluids, other petroleum products, paints, solvents and other deleterious materials must be contained and removed in a manner that will prevent their discharge to waters and soils of the state. The cleanup of spills should take precedence over other work on the site. 10) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance Division. There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be obtained at 1-800-647-09§2,.,The person signing this condition is either the homeowner, agent for the owner or a registered contractor according to WA state law. X 4,%I, I I 11) All approved plans are required to be on-site for inspection purposes. If inspection is called for and plans are not on site, Approval WILL NOT be granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour will be charged and collected by the Mason County Building Department prior to any further inspections being performed or approvals granted. X 12) Owner/Agent is responsible to post the assigned address and/or purchase and post private road signs in accordance with Mason County Title 14.28. �;Z 13) The approved site plan is required to be on-site for inspection purposes. If inspection is called for and the site plan is not on site, Approval WILL NOT be granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour will be charged and collected by the Mason County BuildingjDepartment prior to any further inspections being performed or approvals granted. X� 14) A structural observation, as defined in Chapter 17 of the International Code, shall be provided to the Mason County Building Department. Reports shall be prepared by the engineer or architect of record responsible for the structural design or another engineer or architect designated by the engineer or architect or record. Reports shall be submitted to the Mason County Building Department within 2 weeks following each structural observation. A final observation report shall be submitted prior to the final inspection of the structure. Written statements shall indicate the permit number, type of inspection, and identify any repgged deficiencies. X 15) In addition to the inspections required in IBC, Section 109, the owner, the engineer or architect of record acting as the owner's agent shall employ one or more special inspectors who shall provide inspections during construction on the types of work listed under Chapter 17 and as specified by the design professional. The special inspectors duties & responsibilities shall be as specified in Chapter 17. Special inspection reports shall be submitted to the Mason County Building Department, PO Box 186, Shelton WA 98584 and available for inspection. Inspection reports shall be completed and submitted to the dept. in a timely manner and shall be submitted prior to the framing and final occupanc insjpections. X 2)�`� I COM2008-00114 3 of 5 16) Any changes in construction shall be reviewed by engineer of record and submitted in writing to the Mason County Building Department prior to construction. All engineering documents are a part of the approved set of plans and must remain attached thereto. If engineering documents are removed, approval will not be granted. X - 17) ALL CONSTRUCTION MUST MEET OR EXCEED ALL LOCAL CODES AND THE INTERNATIONAL CODE REQUIREMENTS AND OCCUPANCY IS LIMITED TO THE PERMITTED AND APPROVED CLASSIFICATION. ANY CHANGE OF US OCCUPANCY WOULD RESULT IN PERMIT REVOCATION. CHANGE OF USE MUST BE APPROVED PRIOR TO CHANGE. x 18) Changes to approved building plans that affect compliance to the current Washington State Energy Code (WSEC), ventilation and Indoor Air Quality Code (VI Building/Plumbing/Mechanical Codes and/or Mason County Regulations shall be approved prior to construction. J 19) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE ADOPTED BUILDING CODE. The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason Coun� Build g Inspector shall be made prior to requesting additional inspections. 20) All property lines shall be clearly identified at the time of foundation inspection. X � 21) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-complia;it,vsith Mason County ordinances and building regulations. 22) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit nh Wer have prevented action from being taken. No more than one extension may be granted. X f�9 23) Pressure treated wood manufactured after January 1, 2004 may contain high concentrations of copper which could quickly corrode metal fasteners, connectors, and flashing. Install metal connectors approved for contact with the new types of pressure treated material. This permit becomes null and void if work or construction authorized is not commenced within 180 days,or if construction or work is suspended fora period of 180 days at any time after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied. Proof of continuation of work is by means of a progress inspection.The owner or the agent on the owners behalf, represents that the information provided is accurate and grants employees of Mason County access to the above described property and structure for review and inspection.
